<br />City of Sunny Isles Beach |Request for Qualifications No. 20-07-01 16 <br /> <br />at cost, as the work progresses, or as otherwise agreed upon. Such documentation and <br />records will be available at all reasonable times for examination and audit by the City. <br />Incomplete or incorrect entries in such books and records shall be immediately corrected <br />or completed upon being called to the attention of the Firm. Said books and records for <br />each project shall be kept for a period of five years after the completion of all work to be <br />performed on such project, pursuant to the agreement. <br /> <br />2.14 Payment <br /> <br />The City will make monthly payments to the Firm based on the percentage of completion <br />of consultant's work, or as otherwise agreed upon. The invoice shall include the following <br />information: <br /> <br />a) Invoice number for the Project, Name of Project, and date; <br />b) The Firm’s lump sum or negotiated upset limit fee; <br />c) Percent of work completed, or employees name, titles, direct labor rates, and multiplier; <br />d) Amount earned; <br />e) Amount previously billed; <br />f) Amount due this invoice; <br />g) Retention, if applicable; <br />h) Balance remaining; <br />i) Attached list of reimbursable with appropriate receipts; <br />j) Summary of work done this billing period. <br />When the Project involves bidding and construction or demolition, the Project, and <br />subsequent payments shall be divided into a minimum of three components -Design, <br />Bidding, and Construction. <br /> <br />Invoices should be e-mailed to AccountsPayable@sibfl.net. <br /> <br />2.15 USE OF SUB-CONSULTANT(S) <br /> <br />If one or more sub-consultant are to be used, the sub-consultant must be clearly <br />identified and noted in the submittal when it is submitted. The City must approve any <br />changes in the use of sub-consultant(s) in advance and in writing. No such approval will <br />be construed as making the City a party to such subcontract, or subjecting the City to <br />liability of any kind to any sub-consultant. No subcontractor will under any circumstances <br />relieve the Respondent of its liability and obligation under any resulting contract. <br />Subcontractor is subject to the same contractual conditions as is the Respondent. The <br />successful firm shall be responsible for all payments to any sub-consultants and shall <br />maintain responsibility for all work related to the Project. <br /> <br />
