Respondents must provide at least three (3) <br />references of work within the last (5) years, preferably from government agencies. Do not <br />include in your references any former or current employee of the City. Provide a resume <br />for all proposed personnel on the Respondent’s team that will be assigned to the Contract <br />to meet the requirements of the Solicitation documents. The resume shall concentrate on <br />the person’s experience and qualifications as it relates to the requirements for the services. <br />Concentrate only on those services rendered by the Firm within the past ten (10) years, <br />which mostly match the scope of services. <br /> <br />Please indicate which team members were part of the referenced service and will be a <br />part of the City’s team. Indicate whether the experience was with the current Firm or a <br />past employer. In all illustrations of experience, indicate specifically the nature and extent <br />of the work performed by the individuals or firms on prior similar services. The title and a <br />brief description of each project and/or service shall include: <br /> <br />a. Client Name, Address, Contact, Phone and Fax Numbers and valid e-mail address; <br />b. Project Title; <br />c. Description of work performed, including the project location; and <br />d. Year completed. <br />e. Past 5 years copies of executed agreements made between your firm and local <br />municipalities, with a scope similar to this RFQ. <br /> <br />If sub-consultants are a part of the proposed work plan, include items A through E for <br />each firm, including an organizational chart showing the relationship between the <br />responding firm and the sub firm(s). <br /> <br />G. Corporate Standing and Authorized Signatory <br />Respondent must demonstrate that the company is in good standing and that the person <br />signing this submittal is an Authorized Signatory on behalf of the respondent to sign bids, <br />proposals, negotiate and/or sign contracts, agreements, amendments and related <br />documents to which the respondent will be duly bound. The respondent must provide a <br />copy of the State Certificate of good standing listing the officers of the company. If the <br />signatory is not one of the officers listed on the State Certificate, the respondent must <br />provide one of the following forms of evidence of Signatory Authority with its response: <br /> <br />a. A copy of firm’s Articles of Incorporation listing the approved signatories of the <br />corporation; <br />b. A copy of a resolution listing the members of staff as authorized signatories for <br />the firm; and <br />c. A letter from a corporate officer listing the members of staff that are authorized <br />signatories for the firm. <br />
