Kimley)))Horn <br />Page 5 of 5 <br />Exhibit A to Amendment Number 02 <br />dated December 21, 2020 <br />All permitting, application, and similar project fees will be paid directly by the Client. <br />Fees and expenses will be invoiced monthly based, as applicable, upon the percentage of services <br />completed or actual services performed, and expenses incurred as of the invoice date. Payment will <br />be due within 25 days of your receipt of the invoice. <br />FUTURE NOT IN SCOPE TASKS <br />Not included in the scope of services shown above are permitting tasks that can be provided as <br />additional services if requested by the Client. These tasks consist of: <br />TASK 5 — FOOT PERMITTING ASSISTANCE - $5,000 <br />Kimley-Horn will assist the Client in preparation of the required FDOT/Agency permits associated with <br />improvements within the Collins Avenue right of way. At this time, we anticipate the following permits <br />will be required: <br />■ FOOT ROW Utility Permit <br />■ FOOT Drainage Permit <br />■ FOOT Access Permit <br />This task includes up to 25 -man hours for permitting assistance purposes. Additional man-hours, if <br />needed will be addressed as an additional service with written authorization from the Client. <br />Preparation of permit applications and submittals of the same as well as permit fees are not part of <br />this task. <br />TASK 6 — SIB GOVERNMENT CENTER ON-SITE PERMITTING ASSISTANCE - $8,000 <br />Kimley-Horn will assist the Client in preparation of the required agency permits associated with the <br />inclusion of the bridge within the SIB Government Center site on the west side of Collins Avenue. At <br />this time, we anticipate the following permits will be required: <br />■ DRER Paving and Drainage Plan Review <br />■ City of Sunny Isles Breach Building Department <br />■ City of North Miami Beach Public Works Utility Dry Run- Water Service Relocates <br />This task includes up to 35 -man hours for permitting assistance purposes. Additional man-hours, if <br />needed will be addressed as an additional service with written authorization from the Client. <br />Kimley-Nom shall provide the required plans sets to the Client for processing to the applicable agencies. <br />Agency comments shall be provided to Kimley-Hom and once all agency comments are collected, Kimley- <br />Horn shall provide responses to these comments. Kimley-Horn shall respond to two (2) rounds of <br />comments. <br />Preparation of permit applications and submittals of the same as well as permit fees are not part of <br />this scope of services. All application, review, permit and impact fees are to be paid directly by the <br />City. Permits will be applied for in conformance with the guidelines for the listed entities. This task <br />assumes that the Client or Owner will contract the services of a permit expeditor to coordinate, submit <br />and transmit the required plans and permit documents as well as follow up with the applicable <br />permitting agencies. <br />Kimley-Horn does not guarantee the issuance of permits or approvals. If permits are issued for this project, <br />the conditions and expiration dates are the sole responsibility of the City. Kimley-Horn is not responsible <br />for extending time-limited entitlements or permits. Kimley-Horn may provide services related to permit time <br />extension requests and/or re-application of expired permits as an additional service. <br />
