Insurance Summary 2021 <br />Provider <br />Insurance Type <br />Summary <br />The recommendation is to switch to Cigna. This was <br />the most competitive proposal received. Due to the <br />high claims experience this past year; United <br />Healthcare proposed a rate increase of approximately <br />Cigna <br />Medical <br />7% (annual amount of $2,489,641.08). After extensive <br />Healthcare <br />negotiations with various carriers, we elected to switch <br />to Cigna, who submitted a proposal with a 7% <br />decrease (annual amount of $2,163,557.76.), with no <br />plan changes. Savings of approximately $326,083.32. <br />The recommendation is to switch to Cigna who <br />submitted a proposal with a 14% decrease to the <br />PPO current rates, with no plan changes. Annual <br />Cigna <br />amount of $130,566.72. Savings of approximately <br />Dental <br />Dental <br />$17,956.56. <br />This was the most competitive proposal received. <br />Employees have the option to choose from two (2) <br />plans: DMO or PPO. <br />Eyemed has extended their current rates at no <br />additional cost. Annual amount of $18,436.80 <br />EyeMed <br />Vision <br />We are in our 4th year of a multi-year rate lock. <br />Life; Accidental <br />Mutual of Omaha has extended their current rates at <br />Death & <br />no additional cost. Annual amount of $64,296.58 <br />Mutual of <br />Dismemberment; <br />Omaha <br />Long -Term <br />Disability; <br />Mutual of Omaha has extended their current rates at <br />Voluntary Short <br />no additional cost. Annual amount of $60,000.00 <br />Mutual of <br />Term Disability <br />Omaha <br />and Voluntary <br />Voluntary Short Term Disability and Voluntary Life <br />Life Insurance <br />Insurance is at the sole cost of the employee. <br />
