CRAIG A. SMITH & ASSOCIATES PROPOSED SCOPE OF SERVICES <br />P4009 <br />GENERAL <br />Craig A. Smith & Associates (CAS) proposes to accomplish the services as follows: <br />ITEM <br />NO. <br />-- PAGE 2-- <br />S86 ROUTE SURVEY (BASIN A — 51 ACRES, MORE OR LESS) <br />Survey will physically locate all above ground, visible improvements within the existing <br />rights of way of the following streets: <br />Bay Ave. from 174" St to 183rd St.; 178T" Dr. from Bay Ave. to its intersection with 1781h <br />St.; Church St.; 1771h Dr.; 1781h St. and 179' Dr. from Bay Ave. to Atlantic Blvd.; 1801h <br />Dr., 18111 Dr.; 182"d Dr. and 183rd St. from Bay Ave. to Atlantic Blvd. <br />Survey baseline will be established at 100-foot intervals and cross sections taken to ten <br />(10) feet beyond each right of way at fifty -foot intervals. Plan view elevations will be shown <br />as relative to North American Vertical Datum of 1988 at each section and at pertinent <br />points for facilitation of Engineering Design. All visible, above ground improvements and <br />utilities will be located and shown. Trees 3" or greater will be shown with diameter at <br />breast height (DBH) and common name if known by the Surveyor's agent. A base map will <br />be created in current software parameters and provided to the Engineering Department as <br />well as a Certified Map of Specific Purpose Survey adhering to the Standards of Practice <br />for Surveying, (Chapter 5J — 17) of the Florida Administrative Code, for submittal to <br />entities of interest. <br />$45,670.00 <br />S87 ROUTE SURVEY - UTILITY SURFACE MARKS (BASIN A) <br />Survey will physically locate all utilities as marked on the surface by the Craig A. Smith & <br />Associates Locating Department within the existing rights of way of the following streets: <br />Bay Ave. from 17411 St to 183rd St.; 178T" Dr. from Bay Ave. to its intersection with 178th <br />St.; Church St.; 1771h Dr.; 178th St. and 179th Dr. from Bay Ave. to Atlantic Blvd.; 180th <br />Dr., 181st Dr.; 182"d Dr. and 183rd St. from Bay Ave. to Atlantic Blvd. <br />The information will be added to the Survey prepared under Task S86 above. <br />$13,050.00 <br />S88 ROUTE SURVEY — UTILITY TEST HOLES (SOFT DIGS) (BASIN A) <br />Survey will physically locate the positions of up to 50 utility test holes (soft digs) within the <br />existing rights of way of the following streets: Bay Ave. from 174th St to 1831 St.; 178T" Dr. <br />from Bay Ave. to its intersection with 178th St.; Church St.; 177th Dr.; 178th St. and 1791h <br />Dr. from Bay Ave. to Atlantic Blvd.; 1801h Dr., 181st Dr.; 182"d Dr. and 1831d St. from Bay <br />Ave. to Atlantic Blvd. <br />The information will be shown on the survey prepared under Task S86 above with utility <br />data, i.e. type, size, material, inv., etc., for each based on reports provided to the <br />Surveyor. <br />$4,700.00 <br />S90 PUMP STATIONS AND OUFALLS (BASIN A) <br />Survey will physically locate all above ground, visible improvements within the areas <br />where the two storm water pump stations may be placed; more particularly the corner of <br />181st drive and N.
