City of Sunny Isles Beach |Request for Proposal No. 22-02-01 29 <br />SECTION 5 <br />PROPOSAL FORMAT <br />5.0 FORMAT <br />Proposers must submit (1) original, five (5) copies and (1) USB of the Proposal. The original <br />Proposal must contain an original signature. Be sure that the individual signing the Proposal is <br />authorized to commit the Proposer's organization to the Proposal as submitted. Each page of the <br />Proposal should state the name of the Proposer, the RFP number, and the page number. The City <br />reserves the right to request additional data or material to support Proposals. All material <br />submitted in response to the RFP will become the property of the City. <br />The proposal should address all the points outlined in the request for proposals. While <br />additional information may be presented, the following subjects, item Nos. 1 through 8 in the <br />following section, must be included. They represent the criteria against which the proposal will <br />be evaluated. <br />LABEL EACH SECTION AS NUMBERED <br />The proposal must be in the following format, no more than 15 pages per section (with the <br />exception of the peer review reports). <br />1.Company Information In response to this Proposal, all Proposers must provide the <br />following: <br />Name of Agency/Company (including any "Doing Business As" names), must be <br />active on Sunbiz. <br />Company Location(s) <br />Internet Web Site Address (if any) <br />Details of Entity Business Structure (Corporation, Partnership, LLC) <br />Date Founded <br />List of any outstanding litigation that would threaten the viability of the firm or <br />the performance of this contract <br />Proof of insurance <br />The firm should also list and describe the firm's professional relationships <br />involving the City of Sunny Isles Beach or any of its agencies for the past five (5) <br />years, together with a statement explaining why such relationships do not <br />constitute a conflict of interest relative to performing the proposed audit. <br />The firm must submit a copy of its two (2) most recent external quality control <br />review (peer review) reports within the last five (5) years with government <br />entities. <br />The firm shall provide an affirmative statement which states firm is independent <br />of the City of Sunny Isles Beach as defined by the U.S. General Accounting Office's <br />Government Auditing Standards (1994).
