NV5 cannot be held responsible for <br />damage to below ground structures or utilities that are not identified to us. We specifically <br />request any available information about the location of underground utilities for this project. <br />4. An NV5 representative will layout test locations based on the furnished drawings. NV5 will <br />perform borings in areas that are indicated to be free from underground utilities. <br />5. We note that the site is an active facilitv. We will relv on the client to provide all necessary site <br />contact information for NV5 to efficiently execute the field work. Site contact name and <br />contact information will be required prior to starting our field work. While NV5 will make <br />reasonable efforts to minimize disruption to any site daily activities, it should be understood <br />that NV5 could need to occupy parking and drive areas for execution of the work. Additionally, <br />drilling is an inherently noisy and messy operation. While NV5 will make reasonable efforts to <br />clean the drill locations upon completion of the work, some soil residue could remain at the <br />ground surface. It is our experience that such soiling is typically washed away by rain showers <br />over time. <br />6. Based on our field data we will perform engineering analyses and prepare an engineering <br />report with evaluations and discussions of the geotechnical aspects of the proposed <br />development, and provide geotechnical recommendations for design and construction. <br />Specifically, the report will provide: <br />• Drawings showing test locations, a graphic summary of the generalized subsurface <br />conditions, and boring logs with detailed descriptions of the materials encountered. <br />• Discussion of generalized subsurface conditions at the site including groundwater <br />including hydraulic conductivity. <br />• Discussion of feasible foundation type(s) for the proposed construction. <br />• Design parameters for the recommended foundation type, including vertical and lateral <br />load resistance. <br />• Estimates of foundation settlements. <br />• Modulus of subgrade reaction for any slabs on grade <br />• Recommendations for pavement thickness design <br />• Recommendations for site preparation and grading, including the re -use of site -excavated <br />materials for fill, fill placement and compaction, and slab and pavement subgrade <br />preparation. <br />• Construction considerations including excavation support and dewatering, impacts of <br />existing foundations, and impacts for adjacent structures. <br />The report will be signed electronically and sealed by a professional engineer licensed in the State of <br />Florida. <br />COMPENSATION <br />We can complete the scope of services proposed herein for a lumpsum fee of $2,400. The fee <br />assumes access to the test locations will be provided at no cost to us and that the test locations can <br />be accessed with a regular truck -mounted drill rig.
