Page 6 of 14 <br />11723.04 — Bella Vista Park <br />City of Sunny Isles Beach <br />Usual Open Hole Procedure. Upon completion of the test, the borehole will be abandoned with <br />grout to the ground surface. Test location can be coordinated with the project civil engineer as <br />appropriate. <br />3. For performing the fieldwork, NV5 will contact Sunshine One Call for advice about the location <br />of underground utilities. Additionally, we require that any other available information regarding <br />the location of underground utilities be provided to us. NV5 cannot be held responsible for <br />damage to below ground structures or utilities that are not identified to us. We specifically <br />request any available information about the location of underground utilities for this project. <br />4. An NV5 representative will layout test locations based on the furnished drawings. NV5 will <br />perform borings in areas that are indicated to be free from underground utilities. <br />5. Based on our field data we will perform engineering analyses and prepare an engineer report <br />with evaluations and discussions of the geotechnical aspects of the proposed development, <br />and provide geotechnical recommendations for design and construction. Specifically, the report <br />will provide: <br />• Drawings showing test locations, a graphic summary of the generalized subsurface conditions, <br />and boring logs with detailed descriptions of the materials encountered. <br />• Discussion of generalized subsurface conditions at the site including groundwater including <br />hydraulic conductivity. <br />• Discussion of feasible foundation type(s) for the proposed construction. <br />• Design parameters for the recommended foundation type, including vertical and lateral load <br />resistance, including recommendations for driven piling for the bulkhead and dock. <br />• Estimates of foundation settlements. <br />• Modulus of subgrade reaction for any slabs on grade. <br />• Design parameters for sheet piles including unit weight, angle of internal friction, and earth <br />pressure coefficients <br />• Recommendations for site preparation and grading, including the re -use of site -excavated <br />materials for fill, fill placement and compaction, and slab and pavement subgrade preparation. <br />• Construction considerations including excavation support and dewatering, impacts of existing <br />foundations, and impacts for adjacent structures. <br />The report will be signed electronically and sealed by a professional engineer licensed in the State of <br />Florida. <br />PLANNING SERVICES <br />Task 301 Site Plan Preparation and Processing <br />KEITH will develop up to three (3) Conceptual Design options for City's review. Based on client <br />feedback, up to two (2) revisions will be developed to define the preferred design direction to be <br />moved forward into the Design Development phase. Concept package shall include: <br />a. Illustrative Site Plan(s) <br />b. Supporting Diagrams <br />c. Character Imagery <br />Once the City has selected the preferred concept, KEITH will prepare one (1) dimensioned / detailed <br />Site Plan in accordance with the requirements of the City Land Development Code which will <br />include proposed building locations, vehicular circulation, required landscape areas/buffers, parking <br />requirements, setbacks, dumpster location, fire access and other issues required by City Land <br />Engineering Inspired Design. 433 <br />1 <br />
