(Cummins Cederberg) is pleased to present <br />this revised proposal addendum for our continued environmental consulting services for the <br />proposed seawall and dock improvements at Bella Vista Park located at 500 Sunny Isles Beach <br />Blvd, Sunny Isles Beach, Miami -Dade County, Florida (Project). <br />Project Introduction <br />Keith and Associates, Inc. (Client) contracted Cummins Cederberg Inc. (Cummins Cederberg) to <br />provide marine engineering and environmental consulting services for the proposed seawall at <br />Bella Vista Park (Project). The contracted scope of work assumed that the seawall would be <br />installed at mean high water and would qualify for a Miami -Dade County Division of Environmental <br />Resources Management (DERM) Short Form Class I Permit, a Florida Department of <br />Environmental Protection (FDEP) exemption, and a US Army Corps of Engineers (USAGE) <br />Nationwide 13 Permit verification. The contract also assumed that the DERM biological <br />assessment would be sufficient for DERM, FDEP, and USACE permitting. <br />During the permitting process, the scope of the Project has evolved to include more components, <br />involving more complex permitting than anticipated in our existing contract. The Biological <br />Assessment conducted by DERM on August 15, 2023, did not include an in -water assessment; <br />therefore, it was not accepted by FDEP as satisfying the in -season seagrass and marine resource <br />survey requirement. DERM also observed two white mangroves on-site, which require <br />unanticipated mitigation negotiations. <br />CUMMINS CEDERBERG, INC.
