SECOND AMENDN'TENT TO THE AGREEMENT BETWEEN <br />_ THE CITY OF SUNNY ISLES BEACH AND <br />JOHN CHURCHILL <br />¢° CONTRACT NO. C1314 -091 <br />This Second Amendment to the Agreement between the City of Sunny Isles Beach ( "City") and <br />John Churchill, ( "Contractor ") executed this _ day of . 2014, is made a part of the <br />original Agreement ("Agreement'), approved by Resolution No. 2012 -1936 on July 19, 2012, between <br />the City and Contractor, attached hereto as Attachment "A ", whose Federal Identification r <br />is . The City and Contractor hereby agree as follows: <br />1. OPTION TO RENEWAL. The City hereby exercises its second option to renew the <br />Agreement for one (1) year in accordance with Section 4 of the original Agreement, as more particularly <br />described in Attachment "A ", which is attached hereto and incorporated herein by reference. The term of <br />this renewal shall commence upon approval of this Second Amendment by the City Commission and <br />shall terminate one year thereafter. <br />2. COST. In accordance with Section 5 of the original Agreement, the City shall pay the <br />Contractor an amount not to exceed Thirty Thousand Dollars (530,000.00) for this one (1) year renewal <br />term. <br />3. OTHER PROVISIONS REMAIN Uq EFFECT: Except as specifically modified herein, all <br />terms and conditions of the original Agreement between the parties, dated July 19, 2012, and attached <br />hereto as Attachment "A" shall remain in full force and effect. <br />4.
