RESOLUTION NO. 2015 - 9)4 3g <br /> A RESOLUTION OF THE CITY COMMISSION OF THE CITY OF <br /> SUNNY ISLES BEACH, FLORIDA, APPROVING THE THIRD <br /> AMENDMENT TO THE AGREEMENT WITH JOHN CHURCHILL <br /> FOR FLEET MAINTENANCE, REPAIR SERVICES, AND <br /> PREVENTATIVE MAINTENANCE TO CITY-OWNED BUSES, IN <br /> AN AMOUNT NOT TO EXCEED THIRTY-SEVEN THOUSAND <br /> FIVE HUNDRED DOLLARS($37,500.00),ATTACHED HERETO AS <br /> EXHIBIT "A"; AUTHORIZING THE MAYOR TO EXECUTE SAID <br /> AGREEMENT;AUTHORIZING THE CITY MANAGER TO DO ALL <br /> THINGS NECESSARY TO EFFECTUATE THIS RESOLUTION; <br /> PROVIDING FOR AN EFFECTIVE DATE. <br /> WHEREAS,the City of Sunny Isles Beach is in need of a contractor to provide heating and <br /> air conditioning services to City-owned buses; and <br /> WHEREAS, the City advertised and issued Request for Proposals(RFP)No. 12-05-02 for <br /> Fleet Maintenance and Repair Services, for which two (2) responses were received; and <br /> WHEREAS, on July 19, 2012 via Resolution No. 2012-1936, the City Commission <br /> approved an Agreement with John Churchill for Fleet Maintenance and Repair Services by <br /> providing heating and air conditioning services to City-owned buses, in an amount not to exceed <br /> Thirty Thousand Dollars ($30,000.00); and <br /> WHEREAS, on November 21, 2013 via Resolution No. 2013-2150, the City Commission <br /> approved the First Amendment to the Agreement with John Churchill for Fleet Maintenance and <br /> Repair Services to City-owned buses, in an amount not to exceed Thirty Thousand Dollars <br /> ($30,000.00); and <br /> WHEREAS, on October 16, 2014 via Resolution No. 2014-2315, the City Commission <br /> approved the Second Amendment to the Agreement with John Churchill for Fleet Maintenance and <br /> Repair Services to City-owned buses, in an amount not to exceed Thirty Thousand Dollars <br /> ($30,000.00); and <br /> WHEREAS,John Churchill is qualified,willing and able to provide the desired services on <br /> the terms and conditions set forth, as well as additional services for preventative maintenance to <br /> inspect vehicles for wear and tear and to note any possible problems that would cause downed time; <br /> and <br /> WHEREAS, the City now desires to approve the Third Amendment to the Agreement with <br /> John Churchill to provide said services, in an amount not to exceed Thirty-Seven Thousand Five <br /> Hundred Dollars ($37,500.00), attached hereto as Exhibit "A". <br /> R2015-John Churchill Fleet Maintenance and Repair Srvs 3rd Amd to Agmt Page 1 of 2 <br />
