<br />Item 4:
<br />Testing, Surveying and Miscellaneous / LS: This item shall cover all testing including
<br />but not limited to compaction test/densities, string-lining, boarding, asphalt core samples
<br />and concrete core testing. This item shall also include all land surveying and layout
<br />services required for the project. This item shall also include miscellaneous expenses
<br />such as, but not limited to, reproduction services, As-Built Plans, meetings with the City,
<br />Engineer of Record, the County, Florida Department of Transportation or other governing
<br />entities, delivery services, etc. as needed to complete the project.
<br />Measurement for payment shall be based on the percentage of work completed from
<br />Notice to Proceed to Final Approval by the City as determined by the City.
<br />
<br />Item 5:
<br />Indemnification Allowance / LS:
<br />The indemnification provided above shall obligate the contractor to defend at his own
<br />expense or to provide for such defense, at the city option, any and all claims of liability and
<br />all suits and actions of every name and description that may be brought against the City
<br />excluding only those which allege that the injuries arose out of the sole negligence of the
<br />city, which may result from the operations and activities under this contract whether the
<br />construction operations be performed by the contractor, his subcontractor or by anyone
<br />directly or indirectly employed by either.
<br />Payment of the said specific consideration shall be made at the same time of the payment
<br />of the first progress estimate and the contractor shall acknowledge payment of this
<br />consideration by letter to the Owner after receipt of the progress payment.
<br />
<br />Items 6 to 25 inclusive:
<br />Install Conduit (FPL to furnish conduit) / LF: The price for these items shall include but
<br />not limited to all labor, equipment and materials necessary to install and construct
<br />conduits, fittings with identification tape, blown –thru pull string, end caps in accordance
<br />with these contract documents which include all saw cutting, asphalt and other excavated
<br />materials removal and disposal, sheeting, shoring, bracing demucking, de-watering, any
<br />measures associated with dewatering such as turbidity barrier and sediment control de-
<br />watering permits, abandonment, removal or relocation of existing utilities, bedding
<br />material, trench backfill or additional suitable backfill material, backfill base material,
<br />compacted backfill limerock base material at least 16” thick or as shown on the road
<br />restoration details, compaction, material to be restored to same elevation as existing
<br />asphalt pavement grading, minimum of 1-1/2” ,or as shown on the road restoration details,
<br />of type SP-9.5 Asphalt Fine Mix installed per FDOT standard specification, first lift to be
<br />installed upon restoration of road base material ,sod replacement, regarding of existing
<br />road surfaces, mixing and regarding of existing base material, Soft digs, test pits,
<br />exploratory digging and restoration for existing utilities, repairs to damaged existing
<br />utilities, repairs to irrigation systems, landscape, driveway ,curb and sidewalk restoration
<br />,striping and pavement markings, signalization repair and all other consumables,
<br />appurtenances and incidentals as shown on the plans and specifications and/or necessary
<br />for a complete installation shall be included in the unit price for this pay item. No additional
<br />compensation shall be made for changes in alignment or grade to avoid existing
<br />infrastructure unless the excavation exceed ten(10) feet in depth.
