WHEREAS, on November 19, 2015 via Resolution No. 2015-2495, the City <br /> Commission approved the Fourth Amendment to the Agreement with Calvin Giordano and <br /> Associates, Inc., to provide design modifications to the Pedestrian and Emergency Vehicular <br /> Bridge for Easement concerns, in an amount not to exceed Seventy-Seven Thousand Six <br /> Hundred Fifty-Two Dollars and Seventy-Five Cents ($77,652.75), bringing the total amount not <br /> to exceed to Nine Hundred Eighty-Four Thousand Four Hundred Eighty-Nine Dollars and <br /> Twenty-Five Cents ($984,489.25); and <br /> WHEREAS, the City now needs Design, Survey, and Materials Testing as well as <br /> additional Construction Management and Inspection Services for the Pedestrian and Emergency <br /> Vehicular Bridge, and Calvin Giordano and Associates, Inc. has expressed its ability and desire to <br /> provide these services subject to the terms and conditions contained within the Fifth Amendment <br /> to the Agreement; and <br /> WHEREAS, the City wishes to approve the Fifth Amendment to the Agreement with <br /> Calvin, Giordano, and Associates, Inc., to provide Design, Survey, and Materials Testing as well <br /> as additional Construction Management and Inspection Services for the Pedestrian and <br /> Emergency Vehicular Bridge, in a total amount not to exceed Eight Hundred Thirty-Four <br /> Thousand One Hundred Ninety-Five Dollars ($834,195.00) which includes a 10% contingency, <br /> in substantially the same form attached hereto as Exhibit "A", bringing the total amount not to <br /> exceed to One Million Eight Hundred Eighteen Thousand Six Hundred Eighty-Four Dollars and <br /> Twenty-Five Cents ($1,818,684.25). <br /> NOW THEREFORE, BE IT RESOLVED BY THE CITY COMMISSION OF THE <br /> CITY OF SUNNY ISLES BEACH, FLORIDA, AS FOLLOWS: <br /> Section 1. Approving the Fifth Amendment to the Agreement. The Fifth Amendment to the <br /> Agreement with Calvin, Giordano, and Associates, Inc., to provide Design, Survey, and <br /> Materials Testing as well as additional Construction Management and Inspection Services for <br /> the Pedestrian and Emergency Vehicular Bridge, in a total amount not to exceed Eight Hundred <br /> Thirty-Four Thousand One Hundred Ninety-Five Dollars ($834,195.00), in substantially the <br /> same form attached hereto as Exhibit "A", be and the same, is hereby approved. <br /> Section 2. Authorization of Mayor. The Mayor is hereby authorized to execute said <br /> Agreement. <br /> Section 3. Authorization of City Manager and City Attorney. The City Manager and the City <br /> Attorney are hereby authorized to do all things necessary to effectuate this Resolution. <br /> Section 4. Effective Date.
