(hereinafter
referred to as "Contractor"), wherein Contractor agreed to furnish all labor and material to
clean 146 stormwater drains, within the corporate limits of the City, in the amount of Twenty
Three Thousand Nine Hundred Dollars ($23,900.00.) Pursuant to the service agreement,
Paragraph 2 provides that if the need for additional work under this agreement is required, a
supplemental work order will be submitted for approval by the City.

This will confirm that the National Pollution Discharge Elimination System (NPDES) no
longer allows the City to dump the contents of the stormwater drains and catch basins in any
available location. Debris is now required to be disposed of in a NPDES approved special
dumpsite, which is lined and capable of dealing with any contaminants that may be contained
within those-drains and basins.

Contractor has advised us that this provision will require it to transport materials to a
northern Broward County site, with the expectation that Contractor shall remove between
fifteen (15) and twenty-five (25) loads from our system, at an average cost of Two Hundred
Sixteen Dollars ($216.00) per load. Consequently, this letter will represent an approval for a
change order of up to Five Thousand Dollars ($5,000.00).

By execution of the acknowledgement set forth below, this shall constitute a supplemental
work order in accordance with Paragraph 2 of the service agreement.
