<br />RESOLUTION NO. 2004 - 1 'D <br /> <br />A RESOLUTION OF THE CITY OF SUNNY ISLES BEACH, FLORIDA, <br />APPROVING A CHANGE ORDER IN THE AMOUNT OF NINETY-FIVE <br />THOUSAND TWO HUNDRED NINETY-FIVE DOLLARS ($95,295.00), <br />ATTACHED AS EXHIBIT "A" AND TO THE CONTRACT BY AND <br />BETWEEN THE CITY OF SUNNY ISLES BEACH AND WEITZ COMPANY, <br />FOR CONSTRUCTION OF THE NEW SUNNY ISLES BEACH <br />GOVERNMENT CENTER LOCATED AT 18070 COLLINS AVENUE; <br />AUTHORIZING THE CITY MANAGER TO DO ALL THINGS NECESSARY <br />TO EFFECTUATE SAID CHANGE ORDER; PROVIDING FOR AN <br />EFFECTIVE DATE. <br /> <br />WHEREAS, by Resolution No. 2002-419 the City Commission approved the ranking of <br />construction management firms for the construction of the City Hall and Tony Roma's Restaurant <br />located at 18050-18070 Collins Avenue, wherein the Weitz Company was selected as the number <br />one ranked firm to work in concert with Spillis Candela (the City's architects) whose contract was <br />approved by Resolution 2001-398; and <br /> <br />WHEREAS, the City has made a number of necessary changes to the construction of the <br />new Sunny Isles Beach Government Center located at 18070 Collins Avenue, which was not <br />included in the basic services under the Agreement, and a Change Order (exhibit A) have been <br />submitted to the City by Weitz Company; and <br /> <br />WHEREAS, the change order is for additional conduit, boxes and labor for the purpose of <br />installing additional security cameras and card access readers in areas ofthe City Hall that were not <br />originally contemplated in the original plan and for the painting ofthe interior ofthe parking garage. <br />The additional expenditure is as follows: <br /> <br />A) Provide all additional raceways (conduits, boxes and pull strings), provide all material and <br />labor for a total amount of $48,762.00 <br />B) Applying block filler and paint of the interior of entire parking garage $46,533.00 <br /> <br />For a total change order in the amount of$95,295.00 <br /> <br />WHEREAS, the City, being satisfied with the performance and quality of workmanship of <br />the Weitz Company, wishes to ratify the change order. <br /> <br />NOW THEREFORE, BE IT RESOLVED BY THE CITY COMMISSION OF THE <br />CITY OF SUNNY ISLES BEACH, FLORIDA, AS FOLLOWS: <br /> <br />Section 1. Approval of Change Orders to Contract: The Change Order to Contract by and <br />between the City of Sunny Isles Beach and Weitz, Inc., for additional services for the installation of <br /> <br />Change Orders: Weitz Company <br />For New Government Center Facility <br />
