Each proposal submitted must have clearly marked on the face of the envelope the <br />following information: <br /> <br />"SEALED BID" <br />JANITORIAL SERVICES - GOVERNMENT FACILITIES <br />OPENING DATE AND TIME: FRIDAY, DECEMBER 17, 10:00 A.M. <br />BID #04-10-01 <br /> <br />2. All Proposals shall be submitted on the Bid Form and shall include all requested <br />documentation and a completed Public Entity Crime Form. <br /> <br />3. Mail or deliver ten (10) sets of the complete proposal with authorized original <br />signatures to: <br /> <br />Office of the City Clerk <br />City of Sunny Isles Beach <br />17070 Collins Avenue, Suite 250 <br />Sunny Isles Beach, FL 33160 <br /> <br />4. Faxed proposals will not be accepted. <br /> <br />5. The City reserves the right to accept or reject any or all proposals received. <br /> <br />6. Proposals shall be publicly opened at, and no proposal received later than, 10:00 <br />a.m., Friday, December 17, 2004. <br /> <br />7. A mandatory pre-bid conference is scheduled for 10:00 a.m., Friday, December <br />10, 2004. All contractors planning to submit a bid are required to attend this <br />meeting. <br /> <br />8.
