(PLEASE INCLUDE ONE ORIGINAL SIGNED <br />BID DOCUMENT IN BLUE INK AND ONE PHOTOCOPY <br />OF SIGNED BID DOCUMENT PER ENVELOPE). The face <br />of the envelope should contain the above address, the date <br />and the time of bid opening, and bid number. Bids not submit- <br />ted on attached bid may be rejected. All bids are subject to <br />the conditions specified herein.Those which do not comply <br />with these conditions are subject to rejection. <br /> <br />1. EXECUTION OF BID: A. Bid must contain an original <br />signature of an individual authorized to bind the bidder. <br />Bid must be typed or printed in ink. The ink should be <br />blue ink. All corrections made by bidder to their bid must <br />also be initialed. The bidder's name should also appear <br />on each page of the bid sheet if required. B. No award <br />will be made to a bidder who is delinquent in payment <br />of any taxes, fees, fines, contractural debts, judgments, <br />or any other debts due and owed to the County, or is in <br />default on any contractual or regulatory obligation to the <br />County. By signing and submitting this bid, bidder attests <br />that it is not delinquent in payment of any taxes, fees, <br />fines, contractual debts, judgments or any other debts <br />due and owed to the County, nor is it in default on any <br />contractual or re9.ulatory obligation to the County. In the <br />event the bidder s statement is discovered to be false, <br />bidder will be subject to debarment and the County may <br />terminate any contract it has with bidder.C. Bidder certifies <br />by signing the bid that no principles or corporate officers <br />of the firm were principles or corporate officers in any <br />other firm which may have been suspended or debarred <br />from doing business with Broward County within the last <br />three years, unless so noted in the bid documents. D. <br />By signing this bid, bidder attests that any and all state- <br />ments, oral, written or otherwise, made in support of this <br />bid, are accurate, true and correct. Bidder acknowledges <br />that inaccurate, untruthful, or incorrect statements made <br />in support of this bid may be used by the County as a <br />basis for rejection of this bid, rescission of the award, or <br />termination of the contract. Bidder acknowledges that <br />the termination of the contract because of a determina- <br />tion of an inaccurate. untruthful, or incorrect statement <br />made in support of this bid may also serve as the basis <br />for debarment of bidder pursuant to Section 21-119 of <br />the Broward County Administrative Code. <br /> <br />2.
