<br />identification numbers. <br /> <br />g. If the applicant is a corporation or partnership, the full name of the <br />corporation or partnership and the state of incorporation. Applicant <br />must submit a copy of the articles of incorporation. <br /> <br />h. If the business is a corporation and is to be conducted under another <br />name, the business name and county of registration under F. S. <br />~865.09. Applicant must submit a copy of the fictitious name <br />registration. <br /> <br />ill There has been a site ins,pection of the applicant's business premises. <br /> <br />ill The Zoning and Planning Director has approved and assigned the zoning use <br />classification. <br /> <br />ill The Code Enforcement and Licensing Oq>artment Director has verified <br />compliance with all applicable laws and regulations and has collected all <br />applicable fees due to the City. <br /> <br />(B) Legality of Use. In the event there is a question as to the legality of a use, the Zoning <br />and Planning Director may require affidavits and such other information slhe may deem appropriate <br />or necessary to establish the legality of the use, before a certificate of use will be issued. <br /> <br />(C) Emergency Locator. The application shall contain a section designated "emergency <br />locator." The applicant shall fill out as part of the Certificate of Use application the names, business <br />and residence addresses and residence and business phone numbers of the owner, and the manager or <br />other persons to be notified in 'case of fire or other emergencies. Any changes in such information <br />during the period for which the Certificate of Use is issued shall be made to the Director or his <br />designee, in writing. <br /> <br />(0) Certificate number. All applications and certificates shall be assigned a number, <br />which may be an individual's social security number and a corporation's federal tax number, or such <br />other uniform method of numbering selected by the City Manager or his designee, with the director <br />of management information systems. <br /> <br />(E) Statement of accuracy. The application form shall contain the following language: <br /> <br />"The undersigned has carefuUy reviewed this application and aU information <br />contained herein has been freely and voluntarily provided. AU facts, figures, <br />statements contained in this application are true, correct and complete to the <br />best of my knowledge and belief. The applicant also acknowledges and <br />understands that the issuance of a City Certificate of Use is contingent upon a <br />zoning compliance inspection and in conjunction with the issuance of a <br />Certificate of Occupancy, and City Occupational License. Failure to comply <br />with the City's Ordinances may result in revocation of said Certificate of Use.
