<br /> <br /> <br />Addendum No.4 <br /> <br />CITY OF SUNNY ISLES BEACH <br />RFQ NO. 10-04-01 - Professional Engineering Services <br />RFQ NO. 10-04-02 - Professional Architectural Services <br /> <br />To All Bidders: <br />Bidders for the above-referenced projects shall take note of the following changes, additions, <br />deletions, clarifications, etc. to the Plans and Specifications, which in accordance with the <br />Contract Documents shall become a part of and have precedence over anything shown or <br />described otherwise. <br /> <br />5. Q: There is a discrepancy with the RFQ No. in the Professional Engineering Services <br />proposal for the City of Sunny Isles Beach. On the front, it mentions RFQ No. 10-04-01, <br />however, inside on the page that mentions 1.1 Introduction, 1.2 Time and Date of <br />Submittal, etc., there is another RFQ No. This time, RFQ No. 07-07-01. Which RFQ <br />No. do we use as correct? <br /> <br />A: The correct number is the first one, the 10-04-01. <br /> <br />6. Q: Is SF 330 substituting SF 254 and SF 255? Or one can submit either the 330 or <br />254/255? <br /> <br />A: Either way, the 330 or the 254/255. <br /> <br />7. Q: Item 2 (list of projects for the past 5 years) can be covered through forms SF 330 and <br />254/255: is it going to be considered separately anyways? <br /> <br />A: Please list the relevant projects in the ten pages (though they are not required to <br />be in the same format as the 330/254/255). <br /> <br />8. Q: According to Addendum 3, proof of authorization to transact business in Florida & <br />the financials will count towards 10-page limitation. Please see Q 2 and Q4 on addendum. <br />We had understood at Pre-Proposal that these pages would not count. Please clarify. <br /> <br />A: The authorization themselves are not included in the ten pages, and though the <br />financial statements (tax return, etc.) are not either, a summary of the status of the <br />firm should be. From past experience, it did not typically take the full allotment <br />of pages for firms to make their case. Please focus on the content. <br /> <br />END OF SECTION <br />
