NOTICE <br />QUEST FOR PROPOSALS <br />CITY OF SUNNY ISLES BEACH <br />REQUEST FOR PROPOSALS NO. 01-10-01 <br />EMPLOYEE DENTAL, LIFE, LONG TERM DISABILITY, AND VISION INSURANCE <br />Notice is hereby given that the City of Sunny Isles Beach is requesting <br />proposals for Employee Dental, Life, Long Term Disability, and Vision <br />Insurance. <br />Sealed Bids will be received at City Hall, City of Sunny Isles Beach, 17070 <br />Collins Avenue, Suite #250, Sunny Isles Beach, Florida, 33160, until 3:00 P.M., <br />local time Friday, November 16, 2001, at which time they will be publicly opened <br />and read in the City Hall Commission Chambers. The envelope containing the sealed <br />bid must be clearly marked "IMPORTANT -BID ENCLOSED FOR EMPLOYEE <br />DENTAL; LIFE; LONG TERM DISABILITY, AND VISION [RFP No. 01-10 - <br />Ql-l:. Each proposer must submit one original and ten (10) copies of the complete <br />proposal package. Faxed or e- mailed bids will not be accepted. <br />Proposals, once received, become the property of the City, cannot be withdrawn, <br />unless withdrawal is based on the reasons set forth in the City's procurement <br />ordinance, and will not be returned to the proposers. <br />The City of Sunny Isles Beach reserves the right to accept or reject any or all bids, to <br />waive any informality, irregularity or technicality in any bid, to readvertise for bids, or <br />take any other such actions that may be deemed to be in the best interests of the City. <br />At the discretion of the City, firms submitting proposals may be requested to make <br />oral presentation as part of the evaluation process. <br />If any addenda are issued to the RFP, a good faith attempt will be made to furnish a <br />copy of such addenda to each of the persons or firms who, according to the records <br />of the City, previously received a copy of this RFP. <br />However, prior to submitting the proposal, it shall be the sole responsibility of each <br />proposer to contact the City to determine if addenda were issued and, if so, to obtain <br />such addenda.
