USE OF PROPOSAL FORMS <br />Proposers MUST complete the appropriate Proposal Form(s) included in this RFP. All <br />blanks on the Proposal Form(s) must be completed; if a question or confirmation is not <br />applicable, it should be answered with an "N /A ". Supplemental information may be attached <br />to the Proposal Form(s). Failure to fully complete the appropriate Proposal Form(s) may be <br />cause of rejection of a proposal. Removal of any part of the proposal forms may invalidate <br />proposal. <br />If additional Proposal Form(s) are needed, photocopy or replicate as appropriate, and attach <br />such additional pages to the page on which the question or chart is stated. <br />The signature on the Proposer's Warranty(ies) must be that of an officer, partner or a sole <br />practitioner of the company making the proposal. The original proposal should be clearly <br />marked and should contain an original signature on the Proposer's Warranty. <br />WAIVER AND /OR REJECTION OF PROPOSALS <br />The City reserves the right to waive informalities in any proposals, to reject any and all proposals <br />in whole or in part, with or without cause, and to accept that proposal (if any) which in its <br />individual judgment will be in its best interest. <br />IRREVOCABILITY OF PROPOSAL <br />In consideration of the City allowing the proposer to make a proposal (offer), each proposer <br />agrees by offering a proposal (offer), that such proposal (offer) shall remain open, not subject to <br />revocation and shall be subject to City acceptance until ninety (90) days after the date indicated <br />in the RFP as the date the insurance coverage or service would incert, if accepted by the City. <br />NON - WARRANTY OF REQUEST FOR PROPOSALS <br />Due care and diligence has been exercised in the preparation of the RFP, and all information <br />contained herein is believed to be substantially correct. However, the responsibility for <br />determining the full extent of the exposures to risk and verification of all information therein <br />shall rest solely with those making proposals. Neither the proposed insured nor its <br />representatives shall be responsible for any error or omission in these underwriting submissions, <br />nor for the failure on the part of the Proposers to determine the full extent of the exposures. <br />COMPLIANCE WITH LAWS, RULES AND REGULATIONS <br />Each proposer is responsible for full and complete compliance with all laws, rules and regulation <br />including those of the Department of Insurance, which may be applicable to it. <br />Page 11 of 17 <br />Dental, Life, LTD, Vision RFP.Doc <br />
