<br />CITY OF SUNNY ISLES BEACH
<br />REQUEST FOR PROPOSALS NO. 01.10 -01
<br />EMPLOYEE DENTAL, LIFE, LONG TERM
<br />DISABILITY, AND VISION INSURANCE
<br />Notice is hereby given that the City of Sunny Isles Beach is re-
<br />questing proposals for Employee Dental, Life, Long Term Disability,
<br />and Vision Insurance.
<br />Sealed Bids will be received at City Hall, City of Sunny Isles Beach,
<br />17070 Collins Avenue, Suite #250, Sunny Isles Beach, Florida,
<br />33160, until 3.00 p.m., local time Friday November 16, 2001, at which
<br />time they will be publicly opened and read in the City Hall Commission
<br />Chambers. The envelope containing the sealed bid must be clearly
<br />marked "IMPORTANT -BID ENCLOSED FOR EMPLOYEE DENTAL
<br />LIFE LONG TERM DISABILITY AND VISION [RFP No. 01 -10 -011 ".
<br />Each proposer must submit one original and ten (10) copies of the com-
<br />plete proposal package. Faxed or e- mailed bids will not be accepted.
<br />Proposals, once received, become the property of the City, cannot be
<br />withdrawn, unless withdrawal is based on the reasons set forth in the
<br />City's procurement ordinance, and will not be returned to the proposers.
<br />The City of Sunny Isles Beach reserves the right to accept or reject any
<br />or all bids, to waive any informality, irregularity or technicality in any bid, to
<br />readvertise for bids, or take any other such actions that may be deemed to
<br />be in the best interests of the City. At the discretion of the City, firms sub-
<br />mitting proposals may be requested to make oral presentation as part of
<br />the evaluation process.
<br />If any addenda are issued to the RFP, a good faith attempt will be made
<br />to furnish a copy of such addenda to each of the persons or firms who, ac-
<br />cording to the records of the City, previously received a copy of this
<br />RFP.
<br />However, prior to submitting the proposal, it shall be the sole responsi-
<br />bility of each proposer to contact the City to determine if addenda were is-
<br />sued and, if so, to obtain such addenda. Please contact the Office of the
