To be eligible for selection, interested consultants must submit one (1) original and <br />five (5) sets of the following information: <br />1. Name, address and company provide, including, but not limited to, a <br />business overview, financial state of the business, annual revenue for pat <br />two (2) years, and names and addresses of persons having financial <br />interest in the firm. Must specify type of contractual agreement between <br />firms and include certificate(s) of authorization to offer professional <br />services through the Florida State Board of Engineers, including prime as <br />well as supporting firms. <br />2. Governmental Projects completed or in process for the past five (5) years. <br />3. Proof of authorization to transact business in Florida from the Florida <br />Secretary of State, from prime as well as supporting firms. <br />4. Proposed organization chart identifying professional employees, <br />professional experience and their area of responsibilities in the firm. <br />5. Appropriate current Federal 254 and 255 forms showing the firm data <br />and supporting the firm's ability to perform each category of work <br />required. <br />6. List of client references with contact names and telephone numbers. <br />In order to be considered, firms must submit the above information to the City <br />Clerk's Office, Sunny Isles Beach Government Center, 18070 Collins Avenue, <br />Sunny Isles Beach, FL 33160. <br />Jane A.
