consultants must submit five (5) complete copies of the submittal that include, at a minimum, the following information: 1. Name, address and company, including, but not limited to, a business overview, financial state of the business, annual revenue for past two years, and names and addresses of persons having financial interest in the firm. If sub-consultants are used, the bid must specify the type of contractual agreement between all firms and include certificate(s) of authorization to offer professional services through the Florida State Board of Engineers, including the prime and supporting firms. 2. A list of all projects (including governmental projects) completed or in process for the past five years, and a corresponding list of client references with contact names and telephone numbers. 3. Proof of authorization to transact business in Florida from the Florida Secretary of State, from the prime as well as supporting firms. 4. Appropriate current Federal 254 and 255 forms showing the firm data and supporting the firm's ability to perform each category of work required. 5. The name, title, qualifications, experience, and all contact information of the proposed project manager. 6. A listing of the proposed personnel to be assigned to the project, listing their qualifications and experience. 7. A listing of all permits expected to be necessary, and the issuing agencies that will be involved. 8. A general schedule showing the mileposts for design, and estimates on the construction timetable. 9. An estimate of the total construction costs (not includinq desiqn fees) for the finish project as designed by the firm. 10. The general breakdown of person hours for the completed design of the project (including the time necessary to obtain of all permits). Submittals will be received by the City Clerk until 10:00 a.m.. Wednesday. April 18. 2007. at the Sunny Isles Beach Government Center, Fourth Floor Conference Room, 18070 Collins Avenue, Sunny Isles Beach, FL 33160. Submittals will be opened publicly and read aloud at this time. Submittals may be rejected after this time. The City is under no obligation to return the submittals.
