I <br /> <br />I <br /> <br />RESOLUTION NO. 2011 - J {.p(p is <br /> <br />A RESOLUTION OF THE CITY COMMISSION OF THE CITY OF <br />SUNNY ISLES BEACH, FLORIDA, AWARDING RFP NO. 10-11-01 <br />TO AND ENTERING INTO AN AGREEMENT WITH SFM <br />SERVICE,INC. FOR LANDSCAPE MAINTENANCE SERVICES, IN <br />AN AMOUNT NOT TO EXCEED FIVE HUNDRED EIGHTY <br />THOUSAND DOLLARS ($580,000.00) ANNUALLY, IN <br />SUBST ANTIALL Y THE SAME FORM AS ATTACHED HERETO AS <br />EXHIBIT "A", BASED ON THE PRICING, TERMS, AND <br />CONDITIONS LISTED IN THE BID RESPONSE, ATTACHED <br />HERETO AS ATTACHMENT "A"; AUTHORIZING THE MAYOR <br />TO EXECUTE SAID AGREEMENT; AUTHORIZING THE CITY <br />MANAGER TO DO ALL THINGS NECESSARY TO EFFECTUATE <br />THIS RESOLUION; PROVIDING FOR AN EFFECTIVE DATE. <br /> <br />WHEREAS, the City is in need of a contractor to furnish all materials, equipment and labor <br />for landscape maintenance services in the City of Sunny Isles Beach; and <br /> <br />WHEREAS, the City issued and advertised Request for Proposals No. 10-11-01, for <br />Landscape Maintenance Services, for which five (5) responses were received; and <br /> <br />WHEREAS, staff has determined that the bid response from SFM Service, Inc., attached <br />hereto as Attachment "A", is the lowest responsible and responsive bid; and <br /> <br />WHEREAS, City staff have recommended the bid award, based on a careful review of the <br />line items and consideration for the Contractor's reputation; and <br /> <br />WHEREAS, SFM Service, Inc. has expressed its ability and desire to provide these services; <br /> <br />and <br /> <br />WHEREAS, the City wishes to award RFP No.1 0-11-0 1 to and enter into an agreement with <br />SFM Service, Inc. to provide services on a unit price basis for Landscape Maintenance Services, in <br />an amount not to exceed Five Hundred Eighty Thousand Dollars ($580,000.00) annually, in <br />substantially the same form as attached hereto as Exhibit "A". <br /> <br />NOW THEREFORE, BE IT RESOLVED BY THE CITY COMMISSION OF THE <br />CITY OF SUNNY ISLES BEACH, FLORIDA, AS FOLLOWS: <br /> <br />Section 1. Incorporation of Recitals. The recitals set forth in this resolution are incorporated <br />herein by reference as if fully set forth herein. <br /> <br />Section 2. Award of RFP and Approval of Agreement. The City Commission hereby awards <br />RFP No. 10-11-01 to and enters into an agreement with SFM Services, Inc. for Landscape <br />Maintenance Services, in an amount not to exceed Five Hundred Eighty Thousand Dollars <br /> <br />R20 11- SFM Srv Inc Landscape Maintenance Agrmt (RFP 10-11-01) <br /> <br />Page 1 of2 <br />
