Your application has <br />been processed and your permit number 17174 is valid through February 28, 2012. <br />Enclosed you will find five (5) decal(s) for your permitted vehicle(s). The decal should <br />be affixed permanently on the inside passenger side windshield of all vehicles permitted <br />• to transport waste in Miami -Dade County. Vehicles that do not have a decal <br />permanently affixed to the windshield will not be allowed access to the facilities. <br />In addition, General Haulers with solid waste accounts in unincorporated Miami -Dade <br />County must pay a disposal facility fee on all accounts except recycling and non- <br />recurring C & D pickups.
