Possible preparation of complete construction contract plans and special provisions for <br />the assigned projects <br /> <br />The City contemplates the firm team including (but not limited to) the minimum following <br />disciplines. <br />. Engineers <br />. Planners <br />. Architects <br />. Landscape Architects <br /> <br />The intent is to have the consultants provide a viable holistic approach to the design process. <br />The final product must be economically feasible, it must have community and business buy- <br />in, and it must be aesthetically pleasing. <br /> <br />1.6 SUBMITTAL REQUIREMENTS <br /> <br />All submittals by consultants shall contain no more than fifteen (15) pages addressing the <br />issues in the attached scoring sheet. All other required documentation (except for 5 and 6 <br />below) will not be considered a part of the fifteen (15) pages. Any consultant wishing to <br />provide the services described in Section 1.5 must submit one (1) original and four (4) <br />additional complete copies that include, at a minimum, the following documentation: <br /> <br />1. Name, address and company, including, but not limited to, a business overview, <br />financial state of the business, annual revenue for past two years, and names and <br />addresses of persons having financial interest in the firm. <br /> <br />2. A list of all projects (including government projects) completed or in process for the <br />past five (5) years, and a corresponding list of client references with contact names <br />and telephone numbers. <br /> <br />3. Proof of authorization to transact business in Florida from the Florida Secretary of <br />State, from the prime as well as supporting firms. <br /> <br />4. Appropriate current Federal 330 form showing the firm data and supporting the firm's <br />ability to perform each category of work described. (Note: These forms will not be <br />counted as part of the fifteen (15) page maximum). <br /> <br />5. The name, title, qualifications, experience, and all contact information of the proposed <br />contact person. <br />
