Products and/or Completed Operations Hazard <br />4.12.2.4. Explosion, Collapse and Underground Hazard Coverage <br />4.12.2.5. Broad Form Property Damaged <br />4.12.2.6. Broad Form Contractual Coverage applicable to this specific Contract, including any hold harmless <br />and/or indemnification agreement. <br />4.12.2.7. Personal Injury Coverage with Employee and Contractual Exclusions removed, with minimum <br />limits of coverage equal to those required for Bodily Injury Uability and Property Damage Uability. <br /> <br />4.12.3. Business Automobile Uability with minimum limits of One Million Dollars (1,000,000.00) per occurrence <br />combined single limit for Bodily Injury Uability and Property Damage Uability. Coverage must be afforded on <br />a form no more restrictive than the latest edition of the Business Automobile Uability policy, without <br />restrictive endorsements, as filed by the Insurance Services Office, and must include: <br /> <br />4.12.3.1 Owned Vehicles; <br />4.12.3.2 Hired and Non-Owned Vehicles; <br />4.12.3.3 Employers' Non-Ownership. <br /> <br />4.12.4. Before starting the Work, the Contractor will file and make sure that all certificates of insurance required by <br />this document and by the Contract are in the City's possession. These certificates shall contain a provision <br />that the coverage afforded under the policies will not be canceled or materially changed until at least thirty <br />(30) days prior written notice has been given to the City by certified mail. The City shall be named as an <br />additional insured on the above-referenced policies. <br /> <br />4.12.5. The Contractor agrees that if any part of the Work under the Contract is sublet, they will require the Sub- <br />Contractor(s) to carry insurance as required, and that they will require the Sub-Contractor(s) to fumish to <br />them insurance certificates similar to those required by the City in this section. <br /> <br />4.13. Cancellation and Re-Insurance: If any insurance should be canceled or changed by the insurance <br />company or should any insurance expire during the period of this contract, the Contractor shall be responsible for <br />securing other acceptable insurance to provide the coverage specified in this section to maintain coverage during the <br />life of this Contract. All deductibles must be declared by the Contractor and must be approved by the City. At the <br />option of the City, either the Contractor shall eliminate or reduce such deductible or the Contractor shall procure a <br />Bond, in a form satisfactory to the City, covering the same. <br /> <br />4.14. Contractor's Responsibility: The Contractor shall have the following responsibilities: <br /> <br />4.14.1. The Contractor will not employ any Sub-Contractor, against whom the City may have reasonable objection, <br />nor will the Contractor be required to employ any Sub-Contractor who has been accepted by the City, <br />unless the City determines that there is good cause for doing so. <br /> <br />4.14.2. The Contractor shall be fully responsible for all acts and omissions of their Sub-Contractors and of persons <br />and organizations directly or indirectly employed by them and of persons and organizations for whose acts <br />any of them may be liable to the same extent that they are responsible for the acts and omissions of <br />persons directly employed by them. Nothing in the Contract Documents shall create any contractual <br />relationship between City or Contractor and any Sub-Contractor or other person or organization having a <br />direct contract with Contractor, nor shall it create any obligation on the part of City to payor to see to <br /> <br />14 <br />
