DERM and MDFD may <br />contBin requ;rernents that affect my ability to obtain a required building pennit for my project. <br />even if the zoning application is approved at public bearinIl- I am aware that a Building p-a is <br />required for all constrUction and that I am responsible for obtaining the required permit. all <br />required inspections. the certificate of Use and Occupancy or Certificate of Completion for any <br />and all strUctUteS and additions whether proposed or existing without permits. Additionally I am <br />aware a certificate of Use and Occupancy most be obtained for the use of the propertY. if <br />approved at a zooing gearing and that failure to obtain the required permits and/or certificalell <br />of Completion. Use and Occupancy will result in 1I1e initiation of enforcement action against the <br />oceupant and owner.
