For Add Alternate #1, the Contractor shall provide a price for the rest of the existing paver areas <br />which includes ALL remain blocks COMBINED. Work shall include but shall not be limited to work related to the surface cleaning, <br />preparation, crack filling, surface smoothing, edge leveling, site protection, mobilization, labor, material, and equipment, and other <br />as required to complete the task as per City requirements and manufacturer's recommendations. The intent of the RFP is to ensure <br />that the total sidewalk project is completed along the entire length of westside sidewalk along Collins Avenue. <br />The cost per items and lump sums listed in the Proposal form shall include the total cost to complete the Work including but not <br />limited to materials, labor, equipment, bonds, off -duty police officers, flag -men, permit application preparation, permitting, permit <br />running, work required to accomplish the construction, coordination costs, and field verification, and others as necessary, and as <br />indicated In the drawings and as intended under this RFP, specifications, addenda, MANDATORY pre - Proposal information and <br />discussions, and any other documents provided to the Contractor by the City to ensure a 100% complete job. <br />These are approximate quantities as developed by the City of Sunny Isles Beach Public Works department. They are generally <br />representative of the work that will be required by the Contractor. CONTRACTOR shall ensure that ALL work for the construction of <br />the proposed roadway improvements is completed within the TOTAL price submitted on this Proposal Form unless it can be proven <br />and agreed in writing by the City that additional work was provided. <br />
