RESOLUTION NO. 2017- 24 <br /> A RESOLUTION OF THE CITY COMMISSION OF THE CITY <br /> OF SUNNY ISLES BEACH, FLORIDA, APPROVING CHANGE <br /> ORDER NO. 4 WITH SOUTHERN UNDERGROUND <br /> INDUSTRIES, INC. FOR 172`D STREET DRAINAGE <br /> IMPROVEMENTS, EXTENDING THE PROJECT <br /> COMPLETION DATE FOR 60 DAYS AND INCREASING THE <br /> TOTAL CONTRACT VALUE IN AN AMOUNT NOT TO <br /> EXCEED ONE HUNDRED FORTY-FOUR THOUSAND THREE <br /> HUNDRED EIGHTY-FIVE DOLLARS AND SEVENTY-THREE <br /> CENTS ($144,385.73), ATTACHED HERETO AS EXHIBIT "A'; <br /> AUTHORIZING THE CITY MANAGER TO DO ALL THINGS <br /> NECESSARY TO EFFECTUATE THIS RESOLUTION; <br /> PROVIDING FOR AN EFFECTIVE DATE. <br /> WHEREAS,on May 5, 2016 via Resolution No. 2016-2555 the City awarded Bid No. 16- <br /> 04-01 to and authorized the City Manager to negotiate and enter into an Agreement with Southern <br /> Underground Industries, Inc. to provide services for 172"d Street Drainage Improvements; and <br /> WHEREAS, on January 19, 2017 via Resolution No. 2017-2655 the City approved <br /> Change Order Nos. 2 and 3, which increased the scope of services and extended the project <br /> completion date by 60 days; and <br /> WHEREAS, staff has determined that additional utilities need to be relocated in order to <br /> maximize the efficiency of the drainage system and needs to increase the original scope of work <br /> of the Agreement; and <br /> WHEREAS, the City Commission wishes to approve Change Order No. 4, increasing the <br /> total monetary value of the contract in an amount not to exceed One Hundred Forty-Four Thousand <br /> Three Hundred Eighty-Five Dollars and Seventy-Three Cents ($144,385.73) to cover the costs of <br /> the increased scope of services,bringing the total contract amount not to exceed One Million Three <br /> Hundred •Seven Thousand Six Hundred Ninety-One Dollars and Ninety-Eight Cents <br /> ($1,307,691.98). <br /> NOW THEREFORE, BE IT RESOLVED BY THE CITY COMMISSION OF THE CITY <br /> OF SUNNY ISLES BEACH, FLORIDA, AS FOLLOWS: <br /> Section 1. Approval of Change Order No. 4 to the Agreement. The City Commission <br /> hereby approves Change Order No. 4, extending the completion date by 60 days and increasing <br /> the total monetary value of the contract in an amount not to exceed One Hundred Forty-Four <br /> Thousand Three Hundred Eighty-Five Dollars and Seventy-Three Cents ($144,385.73) to cover <br /> the costs of the increased scope of services, bringing the total contract amount not to exceed One <br /> Million Three Hundred Seven Thousand Six Hundred Ninety-One Dollars and Ninety-Eight Cents <br /> ($1,307,691.98). <br /> Section 2. Authorization of City Manager. The City Manager is hereby authorized to do all <br /> things necessary to effectuate this Resolution. <br /> Section 3. Effective Date.
