I further understand I will be required to have a negative test result before I will be permitted to return to work I hereby authorize the medical and/or specimen collection facility to disclose all pertinent medical information and all laboratory results to a Medical Review Officer("MRO") hired or retained by FirstService ResidentiaL The release by the medical and/or specimen collection facility of the information and results, and the utilization of the information and results by the MRO shall be for a limited purpose of providing FirstService Residential an opportunity to evaluate the results and thereby determine whether I am fit to perform my job or the job for which 1 am considered. I also authorize the MRO to disclose the test results to FirstService Residential's Human Resources Department. The release by the MRO of test results and the utilization of these results by FirstService Residential shall be for the limited purpose of providing FirstService Residential an opportunity to evaluate the results and thereby determine whether I am fit to perform my job or the job for which I am considered. I also voluntarily agree that all information regarding my drug test results may be released by FirstService Residential, its Medical Review Officer (MRO), employee assistance program, laboratory and their agents in any administrative,civil or other legal proceeding to the court or appropriate hearing officer,adjudication or fact finder in which I am a party which relates to my employment and in which the results of my drug test are relevant.
