® Attachment 9—TDSR Closure Checklist <br /> TDSR SITE CLOSURE CHECKLIST <br /> 1. Site number and location—physical address and GPS location <br /> 2. Date closure complete <br /> 3. Household hazardous waste removed <br /> 4. Debris Removal Contractor equipment and temporary structures (i.e., towers) removed <br /> 5. Debris Removal Contractor has remediated petroleum spills <br /> 6. Ash piles removed <br /> 7. Comparison of baseline information to conditions after the Debris Removal Contractor has vacated <br /> the site. <br /> 8. List of all tasks completed to restore site to original pre-TDSR condition including surface <br /> restoration, and entry and exit road access repairs. <br /> 9. Attach all corresponding daily logs, site closure notes, and photos <br /> 10. Additional items and conditions submitted by Florida Division of Environmental Protection <br /> (FDEP) and City Environmental Resource Management (ERM) <br /> 11.
