ADDENDUM # 1 <br />DATE: Friday, August 24, 2018 <br />TO: ALL PLANHOLDERS <br />FROM: Purchasing Agent <br />RFQ NO.: RFQ 18-08-01 Consulting Engineering Services for Golden Shores Utility Undergrounding <br />and Roadway Improvements (CCNA) <br />SUBJECT: Addendum No. 1 <br />SUBMITTAL DEADLINE: Tuesday, September 4, 2018 at 2:30 PM <br />Please attach this addendum to the documents in your possession and include the Acknowledgement <br />of Addenda form with your response incorporated with the Request for Qualifications. <br />Revisions: (Note: Deletions will be indicated by strikethrough, additions will be <br />indicated with bold font): <br />1.Section 3.1 Scope of Services: <br />The firm will incorporate such data with new design information to be provided to the public <br />utility service providers as required. The firm will assist in the conversion of existing overhead <br />utilities into underground services from North Bay Road to Atlantic Boulevard and from 185th <br />street to 191st. The engineering services will also include incorporation of a new community wide <br />(Golden Shores) roadway systems, upgrades to the stormwater system as needed, limited <br />new sidewalks from the entrances of Collins to Atlantic Blvd and along the east side of Atlantic <br />Blvd, new landscaping, restoration of all disrupted facilities and driveways caused by <br />construction activities, and a design of new street lighting. <br />2.Section 5(F) Page 24: <br />Provide a resume for all proposed personnel on the Respondent’s team that will be assigned to the <br />Contract to meet the requirements of the Solicitation documents. The resume shall concentrate on <br />the person’s experience and qualifications as it relates to the requirements for the services. <br />The SF330 Form and/or resume will suffice. <br />3.Section 3.3 and Project Verification Form: <br />Proposer must have provided various utility Underground Consultation Services for projects at <br />least three (3) Projects over the past five (5) years of similar size, nature and complexity with a <br />construction amount exceeding $1,000,000 and provide evidence using the Project Verification <br />Form provided herein. <br />Exhibit Added:
