IN <br />WADETFJM Subconsultant Agreement <br />Subconsultant Agreement <br />THIS AGREEMENT is made as of 20between Wade Trim 8010 Woodland <br />Center Blvd, Suite 1200, Tampa, FL 33614, (PROFESSIONAL) and Craig A. Smith & Associates, 7777 <br />Glades Road, Suite 410, Boca Raton, FL 33434 (SUBCONSULTANT). <br />A. PROFESSIONAL has made an agreement dated , 20_, With Lanzo <br />Construction125 S.E. 5th Court, Deerfield Beach, FL 33441 (OWNER), which is herein referred to as the Prime <br />Agreement and which provides for performance of professional services by PROFESSIONAL in connection <br />With the Project described therein. The terms of the Prime Agreement are incorporated herein by reference. <br />SUBCONSULTANT shall be bound to such portions of the Prime Agreement and related documents as are <br />pertinent to SUBCONSULTANT'S responsibilities hereunder. <br />B. SUBCONSULTANT agrees to perform certain consulting services for PROFESSIONAL as a part of <br />or in relation to the Prime Agreement as follows: (attach Exhibit, setting forth specific details and <br />specifications, if appropriate.) <br />SUBCONSULTANT agrees to abide by the terms and conditions of the Prime Agreement while preforming the <br />tasks associated with those listed in EXHIBIT A. In support of delivering the scope of work in the Prime <br />Agreement, the SUBCONSULTANT shall lead and perform the efforts to deliver 30%, 60%, 90%, and 100% <br />design completion stage documents in the areas described as Streetscape Improvements (excluding <br />1.04.C.5.d - Landscape. Improvements; and 1.04.C.5.e — Irrigation Improvements in the Design Criteria <br />Package (DCP)); Storm Water System. SUBCONSULTANT shall also review/verify/update DCP reports <br />shown in Task 2.1 of Exhibit A and assist in the development of a report describing the findings and/or <br />recommendations discovered during the review of the documents including the preliminary drainage report <br />prepared by Stantec Consulting Services, Inc., the geotechnical soil survey and report prepared by <br />Professional Services Industries (PSI), "Contamination Screening Report" (CESR) prepared by PSI, obtain <br />utility drawings from franchise utility companies and identify areas for recommended soft digs to identify utility <br />conflicts, review the Driveway Harmonization License Agreement between individual homeowners and the City <br />and provide recommended revisions. <br />SUBCONSULTANT shall complete a full, updated survey of Palm and Hibiscus that meets the City's survey <br />standards including finished floor elevation data. <br />SUBCONSULTANT shall complete updated base drawings reflecting current conditions. <br />SUBCONSULTANT shall lead the efforts and prepare permit applications associated with the Prime <br />Agreement including Task 2.7 of EXHIBIT A. <br />SUBCONSULTANT shall attend project related progress meetings and Community Design Review Meetings, <br />as required. <br />Project required deliverables shall be provided by the SUBCONSULTANT in accordance to and in support of <br />the Schedule of Services included in the Prime Agreement. <br />WT116-01 WHITE -OWNER YELLOW -PROFESSIONAL PINK -PENDING CONTRACTS 12/28/09 <br />Imp <br />
