Craig A. Smith & Associates Inc. <br />Date of Loss: 04/02/2018 <br />• Project: City of Hollywood Utility Locating Services <br />• Claimant alleges CAS did not properly mark facilities. <br />• CAS provides utility location services for the City of Hollywood Utilities Department. <br />Plaintiff was performing work within the city and damaged a city outfall pipe. Plaintiff <br />contends CAS is at fault for not marking the facility. However, the plaintiff failed to call in a <br />Sunshine One Call Ticket for the subject location. CAS's position is that it is not at fault due <br />to the fact that a ticket did not exist for the subject location. Suit is pending. <br />Orlanda Macia Claimant <br />• Date of Loss: 10/31/2016 <br />• Project: 174t1 Street, Sunny Isles Beach Construction Observation <br />• Personal Injury <br />• Claimant, while walking in proximity to the construction site, claimed to have debris which <br />was Floating in the air get into their eye causing medical attention. CAS and the contractor <br />were sued, and CAS's insurance company settled for $3,500. <br />Rebecca Watts v. Craig A. Smith & Associates, Inc. <br />• Date of Loss: 08/25/2015 <br />• Project: TECO Power Utility Locates <br />• Personal Injury/Vehicle Accident <br />• CAS utility locator was involved in a vehicle accident with plaintiff. Plaintiff claimed <br />additional injury to existing health issues. Settlement was offered to CAS's insurance <br />carrier which was declined by carrier.
