1 <br /> <br />ADDENDUM # 1 <br />DATE: Friday, July 17, 2020 <br />TO: ALL PLANHOLDERS <br />FROM: Purchasing Administrator <br />BID NO.: RFQ 20-07-01 Continuing Professional Consulting Services (CCNA) <br />SUBJECT: Addendum No. 1 <br />BID SUBMITTAL DEADLINE: Tuesday, August 11, 2020 at 2:30 PM <br /> <br />Please attach this addendum to the documents in your possession and include the Acknowledgement <br />of Addenda form with your response incorporated with the Request for Qualifications. <br /> <br /> Revisions: (Note: Deletions will be indicated by strikethrough, additions will be <br /> indicated with bold font): <br />Q1. We do not have an architecture license, but do have a landscape architecture license <br />and would like to submit for landscape architecture services. Please confirm that a registered <br />Landscape Architecture firm can submit under Category 1—Architects. <br />A1. The City is currently not soliciting for landscape architecture services under this <br />continuing RFQ. <br />Q2. Is LEED accreditation required if the respondent has and includes their professional <br />license or is LEED accreditation required in addition to license? <br />A2. LEED accreditation would be in addition to professional license. <br />Q3. Re: SECTION 1: INSTRUCTIONS TO RESPONDENTS / GENERAL TERMS AND <br />CONDITIONS, 1.24 CERTIFICATE (S) OF INSURANCE: “At the time of Firms submission <br />the Firm must submit certificates of insurance as outlined in the General Conditions section. <br />All required insurances shall name the City of Sunny Isles Beach as additional insured and <br />such insurance shall be issued by companies authorized to issue insurance in the State of <br />Florida.” Do certificates naming the City as an additional insured need to be submitted with <br />the RFQ response or are specimen certificates evidencing proof of coverage acceptable, <br />provided the required certificates are submitted in the event of contract award? <br />A3. Evidence of proof of coverage is acceptable at the time of submittal however, upon <br />contract award the COI must have the City listed as an additional insured. <br />Q4. Section 2.6.1 LICENSING REQUIREMENTS: “At a minimum, Respondents must have <br />at least one (1) team member accredited in the United States Green Building Council <br />Leadership in Energy and Environmental Design (LEED).” Is Envision Sustainability <br />Professional (ENV SP) certification acceptable as a substitute? As civil engineers, this <br />certification is more applicable to our potential scopes of work than LEED. <br />A4. Yes for engineering disciplines. <br />Q5. Section 5.0 FORMAT: E. Qualification Information and Assigned Personnel requests <br />submission of “Form SF330, include Part I and Part II” and F. Firm References and Similar <br />Work Completed requests submission of Qualification Statement, resumes for proposed
