REQUEST FOR QUALIFICATIONS NO. 20-07-01 <br />CONTINUING PROFESSIONAL CONSULTING SERVICES (CCNA) <br /> <br /> <br /> <br /> <br />TAB F: FIRM REFERENCES AND SIMILAR WORK COMPLETED <br /> <br />James Sheddy (continued) <br />Swale. Sidewalk, Drainage and Roadway Improvements | City of Hallandale Beach | Senior Construction <br />Inspector - Mr. Sheddy was the inspector for the ADA connections, sidewalks and drainage for the project. <br />The plans were submitted for review at 60% and 90% completion. The sidewalk was installed to complete the <br />existing neighborhood sidewalk network. Swales were constructed adjacent to the new sidewal ks and <br />exfiltration trench was installed to eliminate nuisance flooding. <br />N-29A Canal Connector Project | Davie, Florida | Senior Construction Inspector <br />The project includes the construction of 1,407 LF of 72” diameter culvert equalizer, connecting the <br />Central Broward Water Control District's (CBWCD) N-29A Canal to the N-22 Canal. The project also <br />includes demolition, culvert & headwall construction, and the restoration of stabilized pathway, sidewalk <br />and asphalt roadway, along with other minor improvements associated with the proposed project. <br />Garden Acres Neighborhood Improvements | Oakland Park, Florida | Senior Construction Inspector <br />Mr. Sheddy was responsible for inspection services for the construction the Garden Acres Neighborhood <br />Improvements. Craven Thompson provided design, permitting and CEI services for the project. The <br />roadway restoration consisted of 4,985 LF of roadway. The drainage system consisted of 2,136 LF of <br />storm sewer, 4,171 LF and of exfiltration trench. The project also included major roadway, water and <br />sanitary sewer improvements. The water improvements consisted of 5,591 LF of 6” diameter main, and <br />962 LF of 8” diameter main. The sanitary improvements consisted of 7,045 LF of 8” diameter sewer, <br />218 LF of 4” diameter forcemain, and 1,044 LF of 6” diameter forcemain, and two lift stations. <br />NW 21st Avenue Improvements | Oakland Park, Florida| Senior Construction Inspector <br />This 1.2-mile roadway project consisted of widening the existing three (3) lane road (north-south and bi- <br />directional turn lane) to a five (5) lane road (2 north-south travel lanes and a bi-directional turn lane). <br />The Project included a 72” Diameter regional storm drain system that will provide flood protection to <br />approximately 500 acres of existing developments. A new outfall was constructed in the C-13 Canal. <br />The work included approximately 5,000 LF of water main and force main installation, curb and gutter, <br />landscaping and irrigation, lighting and signalization. <br />N-17 Culvert Replacement Project | Davie, Florida | Senior Construction Inspector - Craven Thompson <br />finalized the culvert diameter based on the conceptual size developed in the Western C-11 Basin model <br />that we prepared. We designed and prepared paving/grading/drainage plans, details, notes and <br />specifications for the connection of the eastern terminus of the N-29A canal and the N-22 Canal with a <br />culvert. The existing asphaltic concrete pathway near the N-29A Canal was restored at its current <br />location. <br />Floranada Road Traffic Calming & Roadway Improvements | Oakland Park, Florida | Senior Construction <br />Inspector - The City of Oakland Park was seeking to implement traffic calming measures to enhance the <br />safety of pedestrians and automobiles on Floranada Road between Dixie High way and Federal Highway. <br />Improvements surveyed, designed and permitted by Craven Thompson included traffic roundabouts at <br />the roadway intersections with NE 13th Avenue and NE 16th Avenue. Two traffic chokers were installed at <br />NE 15th Avenue and at NE 15th Terrace. <br />NE 29th Place and NE 203rd Street Sidewalk Improvements | Aventura | Senior Construction Inspector <br />Mr. Sheddy was responsible for the inspection of the project which consisted of the addition of a sidewalk <br />from the existing bus stop on NE 29th Pl. (on the west side of the roadway) to the northeast corner of NE <br />29th Ct. at NE 203rd St., including a marked crosswalk across NE 29th Ct. The intersection is currently <br />signalized. The existing signal is maintained by Miami-Dade County and is part of the signal at NE 29th <br />Pl. This scope of services assumed that the installation of a new signalized crosswalk, in concept, is <br />acceptable to all review agencies and that no analyses or studies were required prior to the preparation <br />of plans. <br />F | Page 53
