<br /> <br /> <br />5. Check safety to include compliance with an upgrade of the safety plan and activity hazard analysis. <br />Review the activity analysis with each worker. <br />6. Notify a Government Representative at least 24 hours in advance of the beginning of the initial phase. <br />Separate minutes of this phase shall be prepared by the CQC System Manager and attached to the daily <br />CQC report. The exact location of the initial phase shall be indicated for future reference and comparison <br />with follow-up phases. <br />7. The initial phase will be repeated for each new crew to work onsite, or at any time acceptable specified <br />quality standards are not being met. <br /> <br />Follow Up Inspections: <br />Follow-up inspections will be performed daily or as frequently as necessary to assure continuing compliance with <br />contract requirements, including control testing, until completion of the particular segment of work. <br /> <br />Records: TFR will maintain current records of all inspections and tests performed in the format detailed below, <br />entitled Quality Control Report. This form will provide factual evidence of the required inspections or tests <br />involved, results of inspections or tests, nature of defects, causes for rejection, etc., proposed remedial action, and <br />corrective actions taken. TFR’s records will cover both conforming and defective items and must include a <br />statement that all supplies and materials incorporated in the work are in full compliance with the terms of the <br />contract. Legible copies of these records will be furnished to a Government Representative daily. Daily, TFR’s <br />designated representative will verify records. <br /> <br />Site-Specific Quality Control Plan <br />The site or contract-specific QC Plan must accomplish the complete daily documentation of operations, utilization <br />and production, safety, and variations from normal operations of a debris management operation. It must be in <br />place and approved for use before beginning any field operations, including site preparation. It must be in full <br />compliance with contract requirements. <br /> <br />Multiple forms are usually formatted for the Plan, which covers different activities. A summary general daily <br />form is also usually formatted for the project that is completed by the CQC Officer, and totals of all activities are <br />reported on summary forms. Usually, all originals and one set of copies are delivered to a Government <br />Representative, and TFR keeps two copies of each form. <br /> <br />Quality Control Organization <br />The duties and specific areas of responsibility of the various members of the Quality Control Organization are as <br />follows: <br /> <br />Operations Manager: Advises all supervisory personnel including, but not limited to, Sector superintendents <br />and key foremen assigned to critical areas, of the requirements of the specifications and advises supervisors <br />concerning methods to be used and the degree of workmanship required. <br /> <br />Quality Control Officer: The Quality Control Officer is responsible for the direct supervision of all <br />superintendents and subcontractors and superintendents to ensure that the work is being performed according to <br />the Contract Plans and Specifications and TFR’s Contractor Quality Control Program. He/she will supervise the <br />Quality Control Personnel and advise other job personnel in advance of Quality Control Requirements, submittal <br />material, inspections, etc. of their work. When work does not comply with contract requirements, he will <br />immediately advise the Project Manager and together they will decide what corrective action should be taken. <br />The corrective action taken will be shown in the daily log. The designated Quality Control Supervisor will have <br />full authority in implementing the Quality Control Program and the Safety Program of TFR Enterprises, Inc. <br /> <br />ITB No. 23-04-01 <br />Disaster Debris Management and Disposal Services <br />102
