<br /> <br />15 <br /> <br />CITY OF SUNNY ISLES BEACH <br />18070 Collins Ave. | Sunny Isles Beach, FL 33160 <br />305.792.1707 | sibfl.net | Purchasing@sibfl.net <br />2.15 PERMITS <br /> <br />FOR GROUP 1 - The Contractor must obtain an annual permit for annual maintenance within the <br />State Roadways (Roadway) from the City of Sunny Isles Beach Engineering Right-of-Way Division <br />and the FDOT. If FDOT requires a permit fee, the Contractor shall pay the annual permit fee. <br />Contractor must meet all the Maintenance of Traffic (MOT) standards per the latest FDOT <br />Construction Manual. <br /> <br />Lane closures are to be approved by Public Works-Operations. Any major Roadway intersection <br />that requires significant work, will require the presence of an off-duty police officer. <br /> <br />The Contractor shall give adequate notices to those in control of all properties affected by the <br />maintenance activities. <br /> <br />Contractor shall always have a copy of approved permit on the truck. It is the Contractor’s <br />responsibility to coordinate notice of commencement, and coordinate with the permitting <br />authority having jurisdiction on any requirements given as a conditional approval of the <br />permitting for as-needed jobs. <br />2.16 LABOR, MATERIALS, AND EQUIPMENT SHALL BE SUPPLIED BY THE CONTRACTOR <br /> <br />The Contractor shall furnish the following, including but not limited to, all labor, material, <br />equipment, barricading, MOT inclusive of any required off duty police officers, adequate <br />supervision, and coordination for satisfactory contract performance. When not specifically <br />identified in the technical specifications, such materials and equipment shall be of a suitable type <br />and grade for the purpose and meeting or exceeding all permit or industry standard requirements. <br /> <br />Certain electrical supplies will be purchased and inventoried by the City for use of the Contractor. <br />The City may request quotes from the Contractor, based on the price sheet, for certain as-needed <br />supplies/equipment. <br /> <br />Police officers will be required on an as-needed basis, based on the scope of the project. As of <br />August 2023, the hourly rate for off-duty police officers is $75.00 per hour with a 4-hour minimum <br />for the officer. <br /> <br />2.17 EMERGENCY SERVICES <br /> <br />Emergency Services are defined as services that are not included within Group 1. These services <br />may include, but are not limited to, natural disasters, or car accidents. Rates are in accordance <br />with Group 2 in the price sheet. <br /> <br />The vendor shall provide 24 hours, 7 days a week, emergency service to the City under the <br />contract. The emergency service response time (defined as the time from acknowledged <br />notification to arrival on-site) shall be within four (4) hours after notification by the City. <br /> <br />
