<br />12 <br /> RFP 24-12-01 CITYWIDE ELEVATOR MAINTENANCE <br />CITY OF SUNNY ISLES BEACH <br />18070 Collins Ave. | Sunny Isles Beach, FL 33160 <br />305.792.1707 | sibfl.net | Purchasing@sibfl.net <br /> STATE: Florida Department of Business and Professional Regulation Elevator <br /> Company <br /> <br /> AND <br /> <br />Any required elevator maintenance/repair certification required in Florida and <br />Miami-Dade County along with registration to conduct business in the State of <br />Florida in all required disciplines. <br /> <br />Proposing Firm must provide a copy of the applicable license(s) with Firm’s response. <br /> <br />Employees of the Elevator Contractor must hold a Florida Elevator Certificate of <br />Competency certification. <br /> <br />Proposer must submit all active business licenses, registrations, and certifications as <br />applicable, documenting Proposer is fully licensed to conduct relevant business in the <br />State of Florida. Occupational/business licenses are required as applicable. <br /> <br />b. Proposer must demonstrate a strong documented track record of providing Electrical <br />Maintenance Services, for a minimum of three (3) commercial clients, within the last <br />three (3) years, 2021-2024. <br /> <br />c. The Proposer shall provide reference letters for no less than three (3) clients in which <br />the Proposer served as Prime Firm for projects similar in size and scope within the last <br />three (3) years, 2021-2024. Please note that the references must be for the same <br />projects in response to MQR “b” above. <br /> <br />2.4 TERM OF CONTRACT <br /> <br />This contract will commence upon execution by both parties which shall be effective for two (2) <br />years, succeeding approval of the contract by the City Commission, or designee, unless otherwise <br />stipulated in the PO order. The contract shall be contingent upon the completion and submittal <br />of all required documents. This contract shall remain in effect until the completion of services, <br />provided that the services rendered by Firm during the contract period are satisfactory and that <br />City funding is available as appropriated on an annual basis. However, in the event funding is not <br />appropriated on an annual basis during any particular year within the contract period, the <br />contract may be canceled upon thirty (30) days written notice to Firm. In such an event, the City <br />shall only be responsible for the reasonable value of services performed prior to the effective date <br />of termination. <br /> <br /> 2.4.1 OPTIONS TO RENEW <br /> <br />Prior to, or upon completion, of the first initial two (2) year term, the City shall have the option at <br />its sole discretion to renew the contract for three (3) additional one-year renewals. The <br />contractor agrees not to assign or transfer the contract, or any portion thereof, without written <br />approval of the City of Sunny Isles Beach. <br />
