Docusign Envelope ID: A6158799-C7BC-4838-B581-OC84CDE5OB97 <br />T Y" City of Pembroke Pines <br />1.5.2 Questionnaires <br />TAB 1 - RESALE PRICING & APPROACH (30 POINTS) <br />The requested information below pertaining to Tab 1 shall be submitted through <br />the "Other Completed Documents" section on Bonfire. <br />1. Resale Pricing: Vendor shall provide a detail listing, including brands, <br />types and flavors of all beverage and snack products along with pricing to <br />be offered in all vending machines. These prices are not subject to change <br />unless mutually agreed upon. <br />2. Service Narrative: This is to include a concise narrative with sufficient <br />detail indicating the proposed approach to providing the required services, <br />including a description of the types and qualities of service that would be <br />provided. <br />3. Sub -Contracting: A listing of all sub -contractors, if any, and the portion <br />of the contract they will perform must be submitted with the proposal. <br />4. Maintenance Plan: Please describe your maintenance plan. <br />5. Equipment Details: Bidders shall furnish the manufacturer and model of <br />the equipment bid, including specifications or capacity, and variety of <br />product to be dispensed. Technical data, including pictures are to be <br />included as part of your bid. <br />6. Schedule: Proposer shall provide a timeline that highlights proposed tasks <br />regarding the installation and maintenance of all vending machines at all <br />specified locations. <br />TAB 2 — REFERENCES FORM (10 POINTS): <br />Provide specific examples of similar contracts References should be from the last <br />five years and should be capable of explaining and confirming your firm's <br />capacity to successfully complete the scope of work outlined herein. As part of <br />the proposal evaluation process, the City may conduct an investigation of <br />references, including a record check or consumer affairs complaints. Proposers' <br />submission of a proposal constitutes acknowledgment of the process and consent <br />to investigate. The City is the sole judge in determining Proposers qualifications. <br />In regards to the References Form portion, you will have the ability to enter <br />information for 5 different references including the Reference Contact <br />Information and the specific Project Information. <br />In addition, do not provide references for City of Pembroke Pines projects. <br />1. Previous Experience: <br />a. How many clients have you provided Services for? <br />b.
