Bucket trucks, scissor lifts, <br />and aerial lifts used for pole and palm work; harnesses and tie-offs as required. <br />• Daily safety stand-up before each shift; documented job-hazard analysis (JHA) for each work <br />zone, especially FDOT ROW segments. <br />• PPE issued and enforced: Class 2 high-visibility apparel, hard hats, fall protection, electrical- <br />safety PPE. <br />• MUTCD-compliant Maintenance of Traffic (MOT) plans for any installation in FDOT ROW or City <br />roadways (per RFP §3.2; Addendum 3, Q29). No lane closures permitted during 6:00 AM–9:00 <br />AM or 4:00 PM–7:00 PM. <br />• HazCom program with current SDS library. <br />• Background checks: every Contractor employee will pass the FDLE FCIC/NCIC Level 2 criminal <br />background check facilitated through the City's Human Resources Department at $67.75 per <br />person, prior to commencement of work (RFP §2.10; Addendum 3, Q32). This cost is included in <br />our pricing. <br />• English-speaking on-duty supervisor present at all times during installation, maintenance, and <br />removal (RFP §3.5). <br />• Employees will refrain from smoking in municipal buildings except in designated areas (RFP <br />§3.5). <br />• Emergency contact list — names, positions, phone numbers, and emails of the account manager <br />and field supervisor — will be delivered to the City prior to commencement of work (RFP §3.5). <br />
