Be <br />The Light Decor has not been a party to any pending or recent litigation that would materially affect its <br />ability to perform the services described in this RFP. <br /> <br />1.2 Consumer Complaints (Last Five Years) <br />No complaints have been filed with the State Department of Consumer Affairs, the Better Business <br />Bureau, or any other consumer-protection agency, or in any Florida court (state or federal), against Be <br />The Light Decor within the last five (5) years that concerned the kinds of services requested in this RFP. <br />The required Dispute Disclosure Form is provided in the Affidavits section. <br /> <br />1.3 Proof of Insurance <br />Certificates of Insurance evidencing the coverages required by RFP §2.11 (Comprehensive General <br />Liability, Umbrella, Workers' Compensation, Business Automobile Liability) are attached as Appendix A. <br />Upon award, an updated Certificate naming the City of Sunny Isles Beach as Additional Insured will be <br />issued in compliance with the RFP. See Section 9 of this proposal for the coverage summary. <br />
