The bidder <br />shall be liable. for any damages or loss to the City occasioned by negligence of the bidder <br />(nr his ageno or any person the bidder has designated in the completion of his contrart as <br />a result of the bid. Contractor shall be required to furnish a certified copy of all lie*ns6s, <br />certificates of competency or efher iicensure requirements necessary to preodme his <br />profession as required by Florida,. State Statute, South Florlda Building Code, Dade County, <br />or City of Aventura Code. Contractors must Include current Dade County certificates of <br />competency. These domiments shall be furnished to the City along with the bid response. <br />Failure to furnish these documents or to have required licensure will be grounds for <br />rejecting the hid and forfeiture of the bid bond. <br />4.1.2: The vendor shall furnish to the Finance_ Support Services Director, City ofAventura, <br />2999 N.E. 191st Street, Suite #500, Aventura, Florida 33180, certifleate(s) of insurance <br />which indicate that insurance coverage has been obtained by an insurance company <br />authorized to do business in the State of Florida or otherwise secured in A manner <br />satisfactory to the City in an amount equal to 100°% of the requirements and shall be <br />presented to the City prior to issuance of any contracts) or award(s) dor..ument(s) which <br />meets the requirements as outlined below: Workers compensation insurance for all <br />employees of the vendor as required by Florida Statute 440 as amended. Comprehensive <br />general liability insurance (GCL) in an amount not less than $500,000.00 combined single <br />limit per occur vmce for bodily Injury and property damage. The GCL policies must include <br />premises/operations, producWcompleted operations, broad form contractual and <br />independent contractors coverages.
