<br />,. <br /> <br />. -. ~ <br /> <br />ACCIDENTAL DEATH AND DISMEMBERMENT INSURANCE <br /> <br />BENEFIT. If an Insured Person sustains an accidental bodily injury, and the injury directly causes one of the following <br />osses within 90 days of the date of that injury; then the Company will pay the benefit listed: <br /> <br />LOSS <br /> <br />BENEFIT <br /> <br />Loss of one hand by severance at <br />or above the wrist <br /> <br />One-half the Principal Sum <br /> <br />Loss of one foot by severance at <br />or above the ankle <br /> <br />One-half the Principal Sum <br /> <br />Irrecoverable loss of the sight <br />in one eye <br /> <br />One-half the Principal Sum <br /> <br />Any combination of two or more <br />of the losses listed above <br /> <br />Principal Sum <br /> <br />Loss of life <br /> <br />Principal Sum <br /> <br />The total benefit for all losses resulting from the same accident may not exceed the Principal Sum, The Principal Sum for <br />the Insured Person's classification is shown in the Schedule of Insurance, ' <br /> <br />TO WHOM PAYABLE. Benefits for loss of life will be paid in accord with the Beneficiary Section. All other benefits will be <br />paid to the Insured Person. <br /> <br />LIMITATIONS. Benefits are not payable for any loss to which a contributing cause is: <br />(1) intentional self-inflicted injury or self-destruction; <br />(2) disease, bodily or mental infirmity, or medical or surgical treatment of these; <br />(3) the Insured Person's participation in a riot; <br />(4) duty as a member of any military, naval or air force; <br />(5) war or any act of war, declared or undeclared; <br />(6) the Insured Person's participation in the commission of a felony; <br />(7) use of drugs; except when prescribed by a Physician; <br />(8) voluntary inhalation of gas, including carbon monoxide; <br />(9) travel or flight in any aircraft, including balloons and gliders; except as a fare paying passenger on a <br />regularly scheduled flight; or <br />(10) the Insured Person's driving a vehicle while having an alcohol concentration of .10 grams of alcohol or <br />more per 100 milliliters of blood. <br /> <br />20 <br /> <br />OCC. <br />05101/01 <br /> <br />GLllOI-1490 <br />
