<br />Project No: 01-14122 <br /> <br />EXHIBIT A - SCOPE OF SERVICES <br />181 st Drive Active Park Project <br />Additional Service No.1 0 - Drainage System Evaluation Services <br /> <br />1.0 PROJECT DESCRIPTION <br /> <br />The Project is generally described as follows: Consultant shall provide Services <br />relating to the evaluation of the drainage system for the :t2-acre park known as the <br />181 st Drive Active Park project in Sunny Isles Beach in Miami Dade County, <br />Florida (the "Project"). <br /> <br />2.0 BASIS OF SCOPE <br /> <br />Consultant's invoices for the construction observation services will be <br />submitted to Jorge Vera, Assistant City Manager with a copy to Sabrina <br />Baglieri, the City Program Manager Consultant. <br /> <br />The newly installed park drainage system has been compromised by <br />construction activities of a neighboring site. Several breaches of a <br />holding pond's berm on that neighboring construction site have caused <br />erosion and silting of the park's drainage system. The soil and silt, etc., <br />have infiltrated the system and may have compromised the performance <br />and life-span of the system. <br /> <br />City wishes the Consultant to 'certify' the drainage system. Remediation <br />strategies and procedures will need to be developed to rectify the issue. <br /> <br />The Consultant will assess the damage to the system and provide <br />recommendations to the City. <br /> <br />Negotiation involving the City, the Consultant, and the Contractors may be <br />necessary to find a consensus resolution to this issue. <br /> <br />In a separate agreement, the City allowed the neighboring construction <br />Contractor to gravity feed stormwater into the park's drainage wells. The <br />Contractor has agreed to make any restitution needed to the well to make <br />the well perform as it was prior to t~e stormwater injection. The well was <br />certified by the well contractor prior to the stormwater injection. <br /> <br />Design modifications or revisions to the drainage system may require <br />additional design tasks and fees. <br /> <br />Page 1 of 2 <br /> <br />Initial: ML: <br /> <br />Client: <br />
