Hines, CMC, City Clerk <br /> <br />MEMORANDUM <br /> <br />TO: The Honorable City Commission <br />~~FM <br />FROM: Hans Ottinot, City Attorney <br /> <br />DATE: July 17,2008 <br /> <br />RE: Written Statement Regarding the Collection of Social Security Numbers <br /> <br />RECOMMENDATION: <br /> <br />It is recommended that the City Commission consider the attached resolution establishing a written <br />policy for the collection of social security numbers. <br /> <br />REASONS: <br /> <br />Recently, the Florida Legislature adopted new requirements relating to the collection and <br />dissemination of Social Security numbers by all agencies in Florida. Basically, the law has two <br />primary parts relating to: (I) adopting a written statement on the collection of Social Security <br />numbers and certifying compliance to the Legislature and (2) maintaining a registry of commercial <br />entities making a public records request for Social Security numbers and reporting such activity to <br />the Legislature before January 31 of each year. <br /> <br />In an effort to satisfy the requirements of the Legislature, staff proposes the adoption of the following <br />written statement regarding the collection and dissemination of Social Security numbers: The City <br />of Sunny Isles Beach collects your Social Security number for the following purposes: <br />Classification of Accounts; Identification and Verification; Credit Worthiness; Billing and <br />Payments; Data Collection, Reconciliation, and Tracking; Benefit Processing; and Tax <br />Reporting. Social Security numbers are also used as a unique numeric identifier and may be <br />used for search purposes. <br /> <br />ADDITIONAL INFORMATION <br /> <br />The City Clerk will file the appropriate certifications on behalf of the City and maintain a registry of <br />commercial entities requesting Social Security numbers, in accordance with Florida Statutes.
