FOR INTERNET MAPPING <br />SERVICES AND GIS/DATA AND WEB HOSTING, <br />CONTRACT NO: C0809-006 <br /> <br />THIS CONTRACTUAL AGREEMENT (hereinafter referred to as the "Agreement") <br />is made in triplicate. this I~ ~day of No,,'-e.,\t\-I~ . 2008. by and between <br />the CITY OF SUNNY ISLES BEACH. Florida. (hereinafter rel'erred to as "City"). and <br />CAL VIN, GIORDANO & ASSOCIATES, INC., (hereinafter rcferred to as "Consultant") <br />whose Federal 1.0. # is <br /> <br />RECIT ALS <br /> <br />WHEREAS. the City is in need of an internet mapping service enhancemcnt and Web <br />Hosting: and <br /> <br />WHEREAS. Consultant has expressed the ability and desire to provide these Services <br />subject to the terms and conditions contained in its proposal as more fully described in <br />Attachment "A". which is attached hereto and made a part hereof: and <br /> <br />WHEREAS, the City desires to enter into an Agreement with Consultant to provide the <br />internet mapping service enhancement in an one time total amount not to exceed Ten Thousand <br />Three Hundred Eleven Dollars ($10.311.00) and a monthly total amount not to exceed Six <br />Hundred Fifty-Six Dollars and Twenty-Five Cents ($656.27) for the web hosting services. with a <br />grand total not to exceed Eighteen Thousand One Hundred Eighty-Six Dollars ($18.186.24). <br /> <br />NOW THEREFORE, in consideration oCthe premises and the mutual covenants herein <br />names. the parties agree as follows: <br /> <br />I. RECITALS. The Recitals set forth above are hereby incorporated into this Agrecmcnt <br />and made a part hereoC for reference, <br /> <br />2. SERVICES. Consultant agrees to furnish all labor and materials in a good workmanlike <br />and professional manner and to perform the Services designated in Attachment "A" attached <br />hereto. and incorporated herein by reference. The Services shall be per/armed by Consultant to <br />the full satisf~1Ction of the City. Consultant agrees to have a qualified representative to audit and <br />inspect the Services provided on a regular basis to ensure all Services are bcing performcd in <br />accordance with the City's needs and pursuant to the terms of this Agreement. and shall rcport to <br />the City accordingly. Consultant agrees to immediately inform the City via tclephone and in <br />writing of any problems that could cause damage to the City's property. improvemcnts and <br />persons, Consultant will require its employees to perform their work in a manner befitting the <br />type and scope of work to be performed, In the event that the Consultant f[lils to complete the <br />Services pursuant to the terms of this contract and City must undertake the completion of <br /> <br />COXO<J.OO(, C^I.VIN.
