As you know, the medical market trend for increases are coming in between 12.5% to
14% annually and dental is 13%. This is based on the rising cost of medical services, dental
services, prescription drugs and healthcare administration. Our office has approached the
marketplace for both lines of coverage and with the diligent effort have been able to secure
quotations within that bracket on all policies.

It is our intention to offer a benefit program, based on the City's direction that does not reduce
benefits nor take away the large participating provider list that is now available. It is also our
intention to offer a benefit program for those members that reside outside the Tri-County area or
have children that live outside of the area. It is our general practice to approach multiple carriers
in the marketplace to ensure that we are able to offer the best product that suits our client's needs
with the most competitive pricing.

Health Insurance

The renewal United Healthcare (UHC) and Neighborhood Health Partnership (NHP) was
received on January 22, 2009. Updated claims experience was also received on that date. Due to
the very high claims loss ratio of 101 %, UHC and NHP would offer their most competitive rate
increase of 39,65 %. We had approached Aetna, AvMed, Cigna, BCBS. Humana and Vista on
December 24, 2008 with claims experience through November 2008 for quotations.

Aetna, AvMed and BCBS provided quotations. Humana has declined due to not being
competitive and Cigna and Vista at this point have not finalized either a quote or a declination.
After reviewing the carriers and the plan combinations they are willing to offer, A vMed has been
able to offer the plan closest to the benefits provided under the (UI-IC) and (NHP) contracts with
competitive pricing. Multiple options were provided and two meetings were held with myself,
Samantha Alvarez, Marisol Butcher and CSIB employees, Douglas Haag, Yael Londono and the
CSIB's committee.

5900 N.
